We have attached the following: Underground Storage Tank Permit Application Environmental Logistics, Inc. - Uniform Hazardous Waste Manifest Oilfield Environmental and Compliance, Inc. - Analysis of soil samples Summary of lab results Misc. pictures of the closure The initial permit was for the removal of three UST's of approximately 10,000 gallons each, which were presumed to have contained gasoline. After excavation of the top of the tank, and the review of the tank location and their proximity to a building that was constructed after the tanks were installed, it was requested that the tanks be "closed" by filling the tanks with two -sack cement slurry. On July 23, 2011, the project commenced with the saw cut and removal of concrete overlying the tanks and pipelines. The Pipelines were flushed with water into the tanks and the tanks were emptied of their contents into barrels. The tanks were pressure rinsed and approximately 1000 pounds of 'dry ice' was divided and added to the UST's. The subsequent LEL readings were satisfactory and the pipelines were removed and the tanks were filled with approximately 155 cubic yards of cement slurry. Cement slurry was added to the UST's until overflowing. Soil samples were procured from beneath the dispensers (2) and pipelines in accordance with UT -30. The pipelines and tops of the UST's were backfilled with soil. Soil samples were procured from the areas between the tanks (four locations at the approximate third points) at a depth of approximately 2 feet and six feet below the bottoms of the UST's. The soil samples were delivered to OEC Labs and analyzed for TPH -Gas, BTEX, MTBE, and Lead (Total). UST Closure 7991 White Lane Sparkling Image Car Wash August 30, 2011 Page 2 Odors of hydrocarbons were detected in two of the soil samples, 16 feet and 21 feet, "NE" sample, and from two samples from beneath pipelines, "4 @ 2 feet" and "5 @ 6 feet ". No evidence of soil staining was observed. All sampling was performed with proper containers, iced transport containers, chain of custody, etc. by experienced and trained professionals, in accordance with SW-846. The excavated areas were resurfaced with concrete. Twenty soil samples were taken from the dispenser and pipeline areas. Eight soil samples were obtained from beneath the three UST's. See attached sketch, and the lab result summary below. As can be seen from the lab results summary, there were very minor amounts of Xylenes (#4@2'0.0032 mg /kg), and TPH -Gas ( #5 @6 ' 1.8 mg/kg) in the pipeline and dispenser areas. In addition there were minor amounts of Benzene (NE@16' 0.012mg/kg) and Toluene (NE@16' 0.051 mg /kg and NE@21' 0.0073 mg /kg). The results indicate that very minor amounts of Xylenes, Benzene, Toluene and TPH - Gas were detected at the locations previously indicated.
