E. Comments' Replaced the Regular 1 Leak Detector with a new FX IV, because the original Leak Detector failed to
catch a leak. I also replaced the Super Leak Detector with a new FX I V, because the original Leak Detector failed to catch a
leak. Once the repairs were made, the new FX I V Leak Detectors were re- tested and confirmed operational.
Replaced the Super OPW Drain Valve on the Super Spill Bucket, because the initial Spill Bucket test failed. Once the Drain
Valve was replaced, we ran another Spill Bucket test which passed. I removed 1 gallon of water from the Regular Master
STP sump.

Monitoring System Certification Form: Addendum for Vacuum /Pressure Interstitial Sensors
1. Results of Vacuum/Pressure Monitoring Equipment Testing
This page should be used to document testing and servicing of vacuum and pressure interstitial sensors. A copy of
this form must be included with the Monitoring System Certification Form, which must be provided to the tank
system owner /operator. The owner /operator must submit a copy of the Monitoring System Certification Form to the
local agency regulating UST systems within 30 days oftest date.

Repairs completed: Replaced the Regular 1 FX1V LLD and Super
FX1V LLD, because the original Leak Detectors were unable to catch a leak.
Once the repairs were made the new Leak Detectors were re- tested and
confirmed operational. I replaced the Super OPW Drain Valve, because the
initial Spill Bucket test failed. Once the Drain Valve was replaced I ran
another Spill Bucket test which passed. I removed 1 gallon of water from
the Regular Master STP Sump.
