Ciro S. Dantuono will serve as pyrotechnic chief with Rick Penunuri [CA 1086 -10] as required California - licensed operator and David Yarbrough, Tim Lackey and Brien Carpenter as crew. Enclosed, please find a completed application, permit and fire detail fees, pyrotechnic plot, certificate of insurance, Zenith's BATF and CA GPD, Import/Export and Wholesaler licenses and current vitae for Ciro S. Dantuono. Our crew will have Material Safety Data Sheets onsite, but should you wish to review them beforehand, I would be delighted to fax or email them to you. FIREWORKS ENDORSEMENT This endorsement modifies insurance provide by this policy: Amended Pollution Exclusion 1. Subparagraph f.(1) of paragraph 2., Exclusions, Coverage A— BODILY INJURY AND PROPERTY DAMAGE LIABILITY (Section I— Coverages) is deleted and replaced with the following subparagraph: f. As used in this exclusion, a hostile fire means one which becomes uncontrollable or breaks out from where it was intended to be or the effects resulting from the Named Insured testing pyrotechnic materials. Exclusion: Injury to Leased and Temporary Workers. 7. Subparagraph s. is added to paragraph 2., Exclusions, Coverage A— BODILY INJURY AND PROPERTY DAMAGE LIABILITY (Section I— Coverages) as follows: s. "Bodily injury" to: 1) Any "leased worker" or "temporary worker" or: 2) The spouse, child, parent, brother or sister of that "leased worker" or "temporary worker" as a consequence of (1) above. This exclusion applies: 1) Whether the insured may be liable as an employer or in any other capacity; 2) To any obligation to share damages with or repay someone else who must pay damages because of the injury; and 3) To liability assumed by the insured under an "insured contract."
