Dear Director Wines,
Zenith Pyrotechnology has been secured to provide the indoor pyrotechnics for the World Wrestling
Entertainment's taped "Smackdown /NXT" television production scheduled at the Rabobank Arena on
Tuesday, 16' August 2011 at 6:45 p.m. and we would like to apply for the necessary pyrotechnic
permits.
Ciro S. Dantuono will serve as pyrotechnic chief with Rick Penunuri [CA 1086 -10] as required
California - licensed operator and David Yarbrough, Tim Lackey and Brien Carpenter as crew. Enclosed,
please find a completed application, permit and fire detail fees, pyrotechnic plot, certificate of
insurance, Zenith's BATF and CA GPD, Import/Export and Wholesaler licenses and current vitae for
Ciro S. Dantuono. Our crew will have Material Safety Data Sheets onsite, but should you wish to
review them beforehand, I would be delighted to fax or email them to you.
Products we hope to employ are manufactured by Next F /X, LeMaitre USA and Ultratec Special
Effects, as follows:

Upon our arrival, product is stored in daybox magazines in the venue safe room.
Kindly note that extinguishers are placed as follows:
One extinguisher at each corner of the stage [4 total]
One at pyro control area
One at stage left
One at the ring
One outside venue saferoom
One inside venue saferoom
We utilize a combination of CO2 and H2O extinguishers.
Show time is 6:45 p.m., the production has allotted a 3:OOp.m. to 4:00 p.m. window for required
inspections; please advise if this is convenient for you. We would request that all fire detail fees be
billed directly to World Wrestling Entertainment, Inc. through the venue, if possible.

FIREWORKS ENDORSEMENT
This endorsement modifies insurance provide by this policy:
Amended Pollution Exclusion
1. Subparagraph f.(1) of paragraph 2., Exclusions, Coverage A— BODILY INJURY AND
PROPERTY DAMAGE LIABILITY (Section I— Coverages) is deleted and replaced with
the following subparagraph:
f. Pollution
1) `Bodily injury" or "property damage" arising out of the actual, alleged or
threatened discharge, dispersal, seepage, migration, release or escape of pollutants:
a) At or from any premises, site or location which is or was at any time owned or
occupied by, or rented or loaned to, any insured;
b) At or from any premises, site or location which is or was at any time used by or
for any insured or others for the handling, storage, disposal, processing or
treatment of waste;
c) Which are or were at any time transported, handled, stored, treated, disposed of,
or processed as waste by or for any insured or any person or organization for
whom you may be legally responsible; or
d) At or from any premises, site or location on which any insured or any
contractors or subcontractors working directly or indirectly on any insured's
behalf are performing operations:
i) If the pollutants are brought on or to the premises, site or location in
connection with such operations by such insured, contractor or
subcontractor; or
ii) If the operations are to test for, monitor, clean up, remove, contain, treat,
detoxify or neutralize, or in any way respond to, or assess the effects of
pollutants.
Subparagraph (a) and (d) (i) do not apply to "bodily injury" or "property damage"
arising out of heat, smoke or fumes from a hostile fire or arising out of the use of
any firework or pyrotechnic materials used in connection with the insured's
operations. This above stated exception does not apply to "bodily injury" or
property damage" which results from any firework or pyrotechnic material being
transported, stored, treated, disposed ofor processed as waste; or transported or
stored for others.
As used in this exclusion, a hostile fire means one which becomes uncontrollable or
breaks out from where it was intended to be or the effects resulting from the Named
Insured testing pyrotechnic materials.

2. Subparagraph g.(2) of paragraph 2, Exclusions, Coverage ABODILY INJURY AND
PROPERTY DAMAGED LIABILITY (Section I— Coverages) is deleted and replaced with
the following paragraph:
2) A watercraft you do not own that is not being used to carry persons or property for a
charge;
Additional Insured – Vendors
3. Paragraph 5. is added to Section II —WHO IS AN INSURED as follows:
5. Any person or organization acting as a vendor, but only with respect to "bodily injury" or
property damage" arising out of "your products" which are distributed or sold in the
regular course of the vendor's business; however, the insurance afforded to a vendor does
not apply to:
a. "Bodily injury" or "property damage" for which the vendor is obligated to pay
damages by reason of the assumption of liability in a contract or agreement. This
subparagraph does not apply to liability for damages that the vendor would have in
the absence of the contract or agreement;
b. Any express warranty unauthorized by you;
C. Any physical or chemical change in the product made intentionally by the vendor;
d. Repackaging unless unpacked solely for the purpose of inspection, demonstration,
testing, or the substitution of parts under instructions from the manufacturer, and
then repackaged in the original container;
e. Any failure to make such inspections, adjustments, tests or servicing as the vendor
has agreed to make or normally undertakes to make in the usual course of business,
in connection with the distribution or sale of the products;
f. Demonstration, installation, servicing or repair operations, except such operations
performed at the vendor's premises in connection with the sale of the product;
g. Products which, after distribution or sale by you, have been labeled or relabeled or
used as a container, or any other thing or substance by or for the vendor.
This insurance does not apply to any insured person or organization, from whom you
have acquired such products, or any ingredient, part or container, entering into,
accompanying or containing such products.
Additional Insured – Blanket
4. Paragraph 6. is added to Section H --WHO IS AN INSURED as follows:
6. Notwithstanding any term or condition of this policy to the contrary, any person or
organization required by written contract to have liability coverage for operations
performed by the Named Insured on behalf of such person or organization will qualify as
an additional insured for such coverage as is provided under Coverage A.
In addition, a lessor who leases a location to the Named Insured and is required by
written contract to have liability coverage for operations performed by the Named
Insured on behalf of such lessor will qualify as an additional insured for such coverage as
is provided under Coverage A.

Amendment — Aggregate Limits of Insurance per Display/Locations
5. Paragraph 2. of Section III —LIMITS OF INSURANCE is deleted and replaced with the
following paragraph:
2. The General Aggregate Limit is the most we will pay for the sum of.
a. Medical Expenses under Coverage C;
b. Damages under Coverage A, except damages because of "bodily injury" or
property damage" included in the "products - completed operations hazard "; and
C. Damages under Coverage B.
The General Aggregate Limit applies separately to each location owned or rented by you
and to fireworks displays which take place away from locations owned or rented by you.
Exclusion: Cross Suits
6. The following additional Exclusion is added to the policy and applies both to Coverage A—
BODILY INJURY AND PROPERTY DAMAGE LIABILITY (Section I-- Coverages) and
Coverage B— PERSONAL AND ADVERTISING INJURY LIABILITY (Section I—
Coverages):
This insurance does not apply to any claim or suit by any insured against any other insured
because of "bodily injury ," "property damage," "personal injury" or "advertising injury."
Exclusion: Injury to Leased and Temporary Workers.
7. Subparagraph s. is added to paragraph 2., Exclusions, Coverage A— BODILY INJURY AND
PROPERTY DAMAGE LIABILITY (Section I— Coverages) as follows:
s. "Bodily injury" to:
1) Any "leased worker" or "temporary worker" or:
2) The spouse, child, parent, brother or sister of that "leased worker" or "temporary
worker" as a consequence of (1) above.
This exclusion applies:
1) Whether the insured may be liable as an employer or in any other capacity;
2) To any obligation to share damages with or repay someone else who must pay
damages because of the injury; and
3) To liability assumed by the insured under an "insured contract."
The following additional definitions apply to this exclusion:
1) "Leased worker" means a person leased to the insured by a labor leasing firm
under an agreement between you and the labor leasing firm, to perform duties
related to the conduct of your business.
2) "Temporary worker" means a person who is furnished to you for a period oftime
of less than one year, to support or supplement your work force in special work
situations such as employee absences, temporary skill shortages and seasonal
workloads.`.,
All other terms, conditions, exclusions remain unchanged.
