Page 1 of 1 All June 24, 2007 Crystal Geyser Bakersfield Ammonia Incident Friday, 06/22/2007 OES #07 -3783 2:30 p.m. on Friday June 22, 2007, Production Manager David Wright began running the filler and at approximately 7:15 p.m. he noticed the ammonia warning light flashing in the filler room. David then looked at the light indicators on the Carbo Cooler control panel that would indicate whether or not the ammonia compressors were running and the lights were off indicating the ammonia compressors were not running. David then called mechanic Charles Wright to the Filler Room and David then sent him to the ammonia compressor room to check the status of the ammonia system. Charles came back and told David the ammonia accumulator was flooded. Charles determined this by checking the sight glass, it was over full and the top switch was tripped which turned the compressors off. David told Charles to call James Bartell & Assoc. Charles called the emergency number for James Bartell & Associates and the answering service put him through to the Bartell employee on call, Steve. Steve told Charles that he had not been to our facility and was not familiar with our system so he could not help him on the phone but would call Jim Bartell and would be on his way here. The plant was still bottling at this time. David then noticed the glycol temperature in the filler room had risen to 96 °F. Twenty minutes earlier the temperature was at 65 °F. At that time David decided he would shut down the line and run the filler out of product. By 7:40 p.m. David started cooling down the tea processing equipment. After several minutes of cooling down, David checked the glycol temperature in the filler room again and it was still at 96 °F. David and Charles then went to the compressor room and all appeared to be fine. There was no odor of ammonia in the room. Jim Bartell called the plant and told David he was an hour away. David told him the accumulator had flooded but other than that all appeared to be normal in the compressor room, compressors were not running. Approximately 15 minutes later the audio alarm for the ammonia system went off in the buildings. David and mechanic Pete Zamora then went back to the ammonia compressor room and there was still no odor of ammonia. David went back to the production building and pulled Charles off of lunch to help him figure out what was happening in the ammonia room. They tried resetting the alarm, it did not reset. They checked the temperature reading on the glycol meter beside the glycol storage tank and the meter read 96 °F. They continued to research the problem, David walked over to the filler room and approximately 10 minutes later, Pete went to David and said he could smell ammonia in the compressor room. David and Pete returned to the compressor room but could not tell where the odor was coming from. While they researched the origin of the odor, the smell continually grew stronger until it was almost intolerable to be in the room. David felt the system was in jeopardy Page 1 of 2 and to avoid a major failure David decided to dump the system using the King Valve. David activated the King valve, opened the 3 valves on the emergency dump system and implemented Incident Action Plan. David then evacuated all the employees. As David was preparing to call the Fire Department, they had arrived on the property. Incident Commander David Wright gathered all employees at the designated staging. area at the end of the parking lot, took a-head count to make sure everyone was accounted for. The Fire Department, Hazardous Materials team, medical units and Environmental Health were all on site. OES was called and the incident was reported as required, they assigned an incident number OES#07-3783. From the time the employees were evacuated from the building until approximately 11:50 p.m., no one entered the building. Once the all clear was given by the HAZ MAT team, the few remaining employees were allowed to enter the building to retrieve their possessions and leave. Although strong residual odors were vented from the ammonia room, at no time was any Rquid ammonia released from the system. The system functioned exactly as it was designed. Dee Secara was instructed by Director of Prevention Services, Howard Wines that this facility would not start up until the sparge tank was emptied and refilled with water. Between 12:30 a.m. and 1:20 am. we called attempting to get a hazardous materials company to remove the sparge tank contents but to no avail, we could not do more than leave messages. At 5:30 a.m. Coles Services returned the call and said they could have a truck here by 7:30 a.m.. By 11:30 a.m. all the contents of the sparge tank had been removed, 9650 gallons. Jim Bartell arranged a delivery of anhydrous ammonia from Hills Brothers Chemical Company. The delivery was for 1105 gallons. Conversion calculation to pounds is 1105 gallons x .979 (temp at time of delivery was 76'F)=1081.795 x 5.1464 lbs = 5567.349 pounds delivered. Although our system is a 4500 pound system, Jim Bartell explained the difference was due to the chiller draining all of its contents during the dumping as well as the carbo cooler. For process cooling, an auvnor . system containing up to 4900 pounds of anhydrous ammonia is used. There are 35 employees at this facility, five of which are trained in the use of Self Contained Breathing Apps (SCBA). PRE- F LMERC�ENC_ Y PLANNING An RMPP was requested by this office from the facility on March 26, 1990. The RMPP was originally submitted on October 2, 1990. Citing the requirements of Article 63 of the Uniform Fire Code, the Bakersfield Fire Department, as Administering Agency for the RMPP program, required Crystal Geyser to install an ammonia diffuser system as part of the RMPP implementation steps. A diffusion system was installed in 1991 which would dilute and dump ammonia to the sanitary sewer in the event of an accidental release. eJ'� 4 6 Few z mz vi 0, 6 9 o .1A 4 o e 4 .i/ 7-,m . ,A won /i ie►i fflip i • In 1992, as part of Crystal Geyser's entire plant modification, a modified RWP was required by this office. During this plant modification, a new ammonia diffusion system consisting of a stationary sparge tank was installed in order to absorb any released ammonia into the water stored in the stainless steel sparge tank for that purpose. On Wednesday, August 14, 1996, between the hours of 6 to 10 p.m., a quality control operator began to sanitize the process equipment. However, the employee was unaware that the ammonia compressors were off due to a recent power failure. There was no other equipment that did not start up after the power failure, therefore, the employee was unaware of the ammonia compressors being off. The sanitation process raised the temperature in the system which caused the ammonia pressure relief valves to open. Fortunately, and as a result of the RMPP process and compliance with Article 63 of the Uniform Fire Code, the ammonia pressure relief valves are piped directly into the sparge tank. Approximately 2000 pounds of ammonia was absorbed by 5000 gallons of waxer in the spmge tank. Had the ammaionia been -released into the atmosphere, the zone of annmonia coition +M W exceeding the level Innmediately Dangerous to Life or Health (iDLM would have extended 3110th of a mile beyond the plant and affected several nearby residential housing complexes (see attached offsite consequence maps). Since the release occurred between the hours of 6 to 10 p.m, the housing tracts would have been at their maucimum occupancy levels. Without the RiV,iPP process, this would have been a severe and extremely hazardous material incident. POST RLC.M NT PHASE The amount of ammonia effectively absorbed by the sparge tank produced about a 3% to 4% solution of ammonium hydroxide. Crystal Geyser management consulted with Bakersfield's Fire Department and Wastewater Treatment Division on proper disposal of the absorbed ammonia solution. Wastewater officials recommended that Crystal Geyser discharge the ammonia solution from the sparge tank into the sanitary sewer while diluting SOOK with fresh water. Crystal Geyser will subsequently amend their RMPP to include the installation of indicator lights to warn the operators of ammonia compressor failure.
