Wines, III Director of Prevention Services Issue Date: July 1, 2015 Expiration Date: June 30, 2018 SUMMARY OF UNIFIED PERMIT CONDITIONS IN GENERAL: 1. This permit must be posted or maintained on site at all times. 2. Any questions regarding this permit shall be directed to the: PREVENTION SERVICES DIVISION Bakersfield Fire Department 2101 H Street Bakersfield, CA 93301 661-326-3979 3. You must notify Prevention Services Division within 30 days of any change in ownership. 4. You must comply with the requirements of the California Health and Safety Code (HSC), and California Code of Regulations (CCR), as applicable:  Ch.6.5 HSC and (T22CCR) for generators or treaters of hazardous wastes.  Ch.6.67 HSC and (40 CFR 112) * for aboveground storage of petroleum.  Ch.6.7 HSC and (T23CCR, Ch.16) for underground storage of hazardous substances.  Ch.6.75 HSC and (T23CCR, Ch.18) for petroleum underground storage tank cleanup.  Ch.6.95 HSC and (T19CCR) for hazardous materials response and inventories.  Ch.6.95 HSC and (T19CCR) for accidental release prevention of acutely hazardous materials and risk management plans. * Title 40 Code of Federal Regulations, Part 112 5. You must pay your consolidated annual permit fee and any state service fees, as applicable, for this permit to continue to remain in effect. HAZARDOUS MATERIALS: 1. Immediately report any release or threatened release of a hazardous material if there is a reasonable belief that life, health, safety, or the environment are at risk by calling:  911 (for emergencies only), or  661-326-3979 for Prevention Services Division, and  800-852-7550 State Warning Center 2. Report any changes or additions to your hazardous materials inventory within 30 days of the change to the Prevention Services Division. 3. Any amount of explosives must be reported to the Prevention Services Division on the chemical description page of the Unified Program Consolidated Forms. SPECIFICALLY: HAZARDOUS WASTE: 1. Any amount of a hazardous waste must be reported to Prevention Services Division on the chemical description page of the Unified Program Consolidated Forms. 2. Hazardous wastes shall be properly labeled at all times and properly disposed of in a timely manner, no later than 180 days from accumulating 27 gallons (100 kg) at any time, or within 90 days if you accumulate more than 27 gallons (100 kg) per month. 3. Keep all waste disposal receipts or manifests for three years. UNDERGROUND STORAGE TANKS: 1. You must ensure that both the owner and operator of the tank are provided with a copy of this permit. If you operate but do not own the tank(s), there shall be a written operating agreement with the owner to monitor the tanks. 2. You must maintain a monitoring program which includes a site map and a monitoring and response program appropriate for the design of the tank(s). 3. The monitoring program shall be maintained on site as an attachment to this permit in compliance with state regulations and will be subject to annual review by the Prevention Services Division. ABOVEGROUND STORAGE OF PETROLEUM: 1. All new or modified aboveground storage tanks shall be installed and/or removed under a valid permit issued by the Prevention Services Division. 2. Any aboveground petroleum storage over 1320 gallons in aggregate storage shall prepare a Spill Prevention Control and Countermeasure (SPCC) plan. 3. A copy of the SPCC shall be maintained on site and a copy also forwarded to the Prevention Services Division. CALIFORNIA ACCIDENTAL RELEASE PROGRAM (CalARP): 1. Any facility required to submit a Risk Management Plan shall implement the prevention program listed in the plan for the covered processes on site. 2. The prevention program shall be self-audited by the facility at least once during the term of this permit. The results of the self-audit shall be available to the Prevention Services Division for evaluation. 3. Accidental releases of federally regulated or extremely hazardous substances in reportable quantities shall, in addition to the immediate reporting requirements, also be reported to the EPA at 800-424-8802.
