Box 81882 Bakersfield, Ca. 93308 1.0 Job Information Job Location: Customer Contact: Job Charge Number: Date of Inspection: UL #: 1.1 Inspection Personnel and Qualifications: Motor City Fleet Service Soils Engineering Motor City 7/3/2013 8466633 Elite Inspection & Consulting Inc. examination personnel are qualified and certified in accordance with Elite Inspection's Quality Assurance Program, Procedure QCP -1 & Steel Tank Institute. These procedures meet or exceed the guidelines contained in the American Society for Nondestructive Testing's SNT- TC -IA. 1.2 Introduction: Motor City Fleet Service has contracted Elite Inspection & Consulting Inc. to perform a UT & Visual Inspection to a Waste Oil Storage Tank located at Motor City Fleet Service. The Storage Tank was removed from service and lifted into position with a fork lift at time of inspection. Ultrasonic Thickness measurements were taken every G' apart in each direction. ( Please See Diagram & UT Readings Page For initial Measurements & Locations) 2.0 Scope of Work: The primary goal of this inspection is to provide your facility with an assessment of the equipment condition of the storage tank. The resultant report shall contain the required information to assess the general condition of the equipment. The Ultrasonic Thickness and Visual inspection methods were used to assess the equipment condition. These methods were used externally and provided data relating to the present condition of the equipment. 3.0 NDE Equipment Used Ultrasonic Manufacturer: Panametrics Model #: 37 DI, Plus Serial #: S/N -081649109 Calibration Block Model #: Steel Serial #: 056242 Transducers Manufacturer: Panametrics Model 9: D790 -SNI Serial #: 694259 MHz: 5 Diameter: 0.312 3.1 Calibration: Calibration for the thickness measurements was clone using a flat carbon steel step wedge measuring .100" —.500". Calibration was performed prior to and after inspection and verified every hour for all inspections according to Elite Inspection & Consulting Inc.'s procedures. 4.0 Inspection Findings 4.1 UT Findings: After performing Ultrasonic Thickness Readings to each TMI. it was noted that no defects were found throughout as a result of consistant baseline UT readings ranging from 0.093" to 0.111" with an overall average of 0.101" throughout. 4.2 Visual Inspection Findings: After performing the Visual Inspection no points of interest were noted defected. This tank meets hazardous waste requirements. 4.3 Conclusion: In brief, Article 9 of CCR, Title 22 section 66260.10 states that containers (including portable tanks) that are used for the accumulation of used oil must be kept in good condition and have adaquate structural support to contain the used oil. There must be no severe rusting, no apparent structural defects or deterioration, and no leaks. All containers must have tight fitting lids and kept closed except when used oil is being added or removed. If a funnel is being used in the bung hole of the container, it must either be removed when the container is not being added to and container closed or equiped with a valve or cover of some sort to prevent leakage. Regular inspection and routine maintenance of all containers is required and faulty containers repaired or replaced. This inspection performed has met all of said criteria found in CCR, Title 22, section 66260.10 Article 9. 5.0 API 12R1 Repair Recommendations: STI Required Recommedations: None necessary at time of inspection. Under no circumstances shall such services extend beyond the performance of the requested inspection of specific equipment and the preparation of reports or similar documents reflecting the inspection data obtained or the opinion formulated on the basis of such inspection. It is expressly understood that all descriptions, comments and expressions of opinion reflect the opinion or observations of the examiner and are not intended nor can they be construed as representations or warranties as to the actual circumstances. Elite Inspection is not assuming any responsibilities of the owner /operator and the owner /operator retains complete responsibility for the engineering, repair and use decisions as a result of the inspection data or other information provided by Elite Inspection. In no event shall Elite Inspection's liability in respect of the services referred to herein exceed the amount paid for such services. Standard of Care: In performing the services provided, Elite Inspection uses the degree, care and skill ordinarily exercised under similar circumstances by others performing such services in the same or similar locality. No other warranty, expressed or implied, is made or intended by Elite Inspection. Limitations of Liability: Nothing in this Agreement shall be construed to mean that Elite Inspection assumes any liability on account of injury to persons or property, including death, except those directly caused by negligent acts of Elite Inspection or its employees and that Purchaser's own responsibility for injury to persons or properties while on or about Purchaser's equipment is in no way affected by this Agreement. Elite Inspection shall not be held responsible or liable for any loss, damage, detention or delay caused by accidents, strikes, lock -outs, fire, flood, acts of civil or military authorities, or by insurrection or riot, or by any other cause that is unavoidable or beyond Elite Inspection's control or in any event for consequential damages.
