Dear Ernie:
On behalf of Costco Wholesale, Barghausen Consulting Engineers, Inc. is submitting documentation for
the installation of the shroud vent box (protective cover for the vent risers) at the above - referenced
Costco facility. The contractor, WSK and Sons, Inc., has been selected as the contractor to complete this
installation. The following items are enclosed for Shane Gardner's review and approval:
1. One (1) Billing & Permit Statement form (Minor Modification)
2. One (1) Underground Storage Tank Permit Application (Minor Modification)
3. One (1) Barghausen Consulting Engineers, Inc. check in the amount of $167.00, made
payable to the City of Bakersfield (Minor Modification fee determination amount provided by
Environmental Officer Shane Gardner)
4. Two (2) copies of the Contractor Certification Packet
5. Four (4) copies of the Site Map /Detail Sheet (30- by 42 -inch format) prepared by Barghausen
Consulting Engineers, Inc., dated June 6, 2016
Please note, the shroud vent box installation does not change the information on the UST Monitoring
Plan; therefore, revisions have not been made in CERS.
