DATED SEPTEMBER 6, 2013 AND SOILS ABSORPTION EVALUATION BY KRAZAN & ASSOCIATES, INC. DATED DECEMBER 2, 2013 2. THE DESIGN ENGINEER SHALL EXERCISE SUFFICIENT SUPERVISOR CONTROL DURING GRADING AND CONSTRUCTION TO INSURE COMPLIANCE W17H THE PLANS, SOILS REPORT, SPECIFICATIONS, AND CODE WITHIN THEIR PURVIEW. J. CIVIL ENGINEER, GEOTECHNICAL ENGINEER, AND BUILDING OFFICIAL WILL BE NO77FIED FORTY EIGHT (48) HOURS PRIOR TO PLACING ANY FILL MA TER/AL. 4. CONTRACTOR AGREES THAT HE SHALL ASSUME SOLE AND COMPLETE RESPONSIBILITY FOR JOB SITE CONDITIONS DURING THE COURSE OF CONSTRUCTION OF THIS PROJECT. INCLUDING SAFETY OF ALL PERSONS AND PROPERTY; THAT THIS REQUIREMENT SHALL APPLY CONTINUOUSLY AND NOT BE LIM17ED TO NORMAL WORKING HOURS; AND THAT THE CONTRACTOR SHALL DEFEND, INDEMNIFY, AND HOLD THE OWNER, ARCHITECT, AND THE ENGINEER HARMLESS FROM ANY LIABILITY, REAL OR ALLEGED, IN CONNECTION WITH 7HE PERFORMANCE OF WORK ON THIS PROJECT, EXCEPTING FOR LIABILITY ARISING FROM THE SOLE NEGLIGENCE OF THE OWNER, ARCHITECT, OR 7HE ENGINEER. 5. THE GRADING CONTRACTOR SHALL CONTACT ALL COMPANIES WITH UNDERGROUND FACIL177ES PRIOR TO BEGINNING CONSTRUCT70N AND VERIFY THE LOCATION AND DEP7H OF ALL UNDERGROUND FACILITIES, INCLUDING TELEPHONE, ELECTRIC, WATER, SEWER, OIL AND GAS LINES. THE ENGINEER ASSUMES NO RESPONSIBILITY FOR BURIED LINES NOT INDICATED ON THE PLAN OR FOR INFORMATION OBTAINED FROM OUTSIDE SOURCES. (USA 1-800-227-2600) 6. THE GRADING CONTRACTOR SHALL BE RESPONSIBLE FOR GRADING ALL AREAS TO + OR - 0.10 FOOT. IF AN AREA SHOULD BE FOUND TO BE MORE THAN 0.10 FOOT OUT OF LEVEL AFTER COMPACTING AND ACCEPTANCE OF GRADING, INE CONTRACTOR SHALL RETURN AND CORRECT THE GRADING AT NO COST TO THE OWNER. 7. THE GRADING CONTRACTOR SHALL WATER AS REQUIRED DURING THE GRADING OPERA TIONS TO PREVENT THE OCCURRENCE OF A DUST NUISANCE AND SHALL PR07ECT CURBS AND 07HER OBJECTS WHICH ARE TO REMAIN. 8. EXCA VA TION - EXCA VA TION SHALL CONSIST OF ALL EXCA VA TION INVOLVED /N GRADING THE PROJECT AS SHOWN ON THE PLANS, EXCEPT ANY EXCA VA TION SEPARA TEL Y DES/GNA TED AND PAID FOR AS A SEPARATE /TEM. 9. THE WORK EMBRACED HEREIN SHALL BE IN ACCORDANCE WITH THE STANDARD SPECIFICATIONS OF THE STATE OF CALIFORNIA DEPARTMENT OF TRANSPORTATION DATED JULY 1999, INSOFAR AS THE SAME MAY APPLY /N ACCORDANCE WITH THE N07ES HEREON. IN CASE OF CONFLICT WITH THE STANDARD SPECIFICATIONS AND ANY N07ES HEREON, THE NOTES HEREON SHALL TAKE PRECEDENCE OVER AND BE USED /N LIEU OF SUCH CONFLIC77NG PORTIONS. SAID SPECIFfCA77ONS SHALL APPLY BUT NOT BE LIMITED TO THE FOLLOWING: A) ALL CONCRETE SHALL BE CLASS B" AS /N ACCORDANCE W17H SECTION 90 AND SHALL HA VE A T LEAST 2500 PSI COMPRESSIVE STRENGTH AT 28 DAYS. B) ASPHAL 77C CONCRETE- SHALL BE TYPE B" MEDIUM GRADED, 1/2" MAXIMUM AGGREGATE AND INTIMATELY MIXED WITH 5-7% ASPHALT. 10. IN THE EVENT CONSTRUCTION STAKING BASED ON MCINTOSH & ASSOCIATES' PLANS, DRAWINGS, OR OTHER DOCUMENTS IS PERFORMED BY ANOTHER FIRM, McINTOSH & ASSOCIATES SHALL BE HELD HARMLESS AND RELEASED FROM ALL LIABILITY ARISING FROM THE USE OF SAID PLANS, DRAWINGS, OR 07HER DOCUMENTS 11. McINTOSH & ASSOCIATES SHALL NOT BE RESPONSIBLE OR LIABLE FOR UNAUTHORIZED CHANGES TO, OR USES OF, THESE PLANS. ALL CHANGES TO THESE PLANS MUST BE APPROVED, IN WRITING, BY McINTOSH & ASSOCIATES. 12. ALL GRADING SHALL CONFORM WITH APPENDIX J - CALIFORNIA BUILDING CODE AND STANDARDS PERTAINING THERE OF AND TO THE RECOMMENDATIONS CONTAINED /N, AND MADE A PART HEREOF, THE PRELIMINARY SOILS REPORT (NOTE 1), AND ALL ADDENDUMS THERETO. IN CASE OF A CONFLICT BETWEEN THE PLANS AND SPECIFICATIONS 7HE MORE STRINGENT REQUIREMENTS WILL TAKE PRECEDENCE. 13 UPON COMPLETION OF GRADING AND BEFORE 7HE START OF CONSTRUCTION, A FINAL SOILS REPORT SHALL BE PREPARED BY 7HE SOIL ENGINEER. 14. THE SOIL ENGINEER SHALL REVIEW ALL EXCAVATIONS PRIOR TO BACKFILLING AND SHALL BE N077FIED OF ANY ITEM ENCOUNTERED DURING THE GRADING OPERATIONS THAT MIGHT AFFECT FOUNDATION STABILITY SO RECOMMENDATIONS CAN BE MADE BY THE SOIL ENGINEER. 15. CUT AND FILL SLOPES SHALL NOT BE STEEPER THAN 2: 1. 16. ALL SLOPES GREATER THAN 77-IREE (3) FEET IN VER77CAL HEIGHT SHALL BE PREPARED AND MAINTAINED TO PREVENT EROSION. IMPORTED FILL MATERIAL SHALL BE IN CONFORMANCE WITH THE RECOMMENDATIONS OF THE SOILS REPORT (NOTE 1) AND SHOULD CONSIST OF ESSEN77ALLY GRANULAR, SILTY SANDS WITH LOW EXPANSION POTENTIAL AND FREE OF GRASSES, WEEDS, ROCKS LARGER THAN FOUR INCHES IN DIAMETER, DEBRIS, AND SOLUBLE SULFATES IN EXCESS OF 1000 PARTS PER MILLION. 17. CLEARING AND GRUBBING - REMOVE ALL DEBRIS, SUCH AS METAL, TRASH, BROKEN CONCRETE, VEGETATION, OTHER BIODEGRADABLE SUBSTANCES, AND UNSUITABLE SOIL FROM AREAS TO BE GRADED. UNSUITABLE SOIL /S SOIL THAT, IN THE OPINION OF THE BUILDING OFFICIAL, SOIL ENGINEER, OR CIVIL ENGINEER, IS NOT COMPETENT TO SUPPORT OTHER SOIL OR STRUCTURES, OR TO SA 77SFACTORIL Y PERFORM ANY OTHER FUNCTIONS FOR WHICH THE SOIL IS INTENDED. 18. ALL FILL TO BE COMPACTED TO A MINIMUM OF NINETY (90%) PERCENT MAXIMUM DENSITY AS DETERMINED BY APPROVAL METHOD PER APPENDIX J OF THE CURRENT CALIFORNIA BUILDING CODE AND CERTIFIED BY TESTS AND REPORT FROM SOILS ENGINEER. 19. ENGINEERED FILL SHALL BE PLACED IN LAYERS NOT EXCEEDING SIX (6) INCHES IN COMPACTED THICKNESS BROUGHT TO NEAR THE OP77MUM MOISTURE CONTENT OR TO A MOISTURE CONTENT COMMENSURATE WITH EFFECTIVE COMPACTION AND SOIL STABILITY, AND COMPACTED TO 90% OF THE MAXIMUM DENSITY PER ASTM D1557-78, UNLESS SPECIFIED OTHERWISE, COMPACTION SHALL BE CERTIFIED BY TESTS AND REPORT FROM THE SOILS ENGINEER. 20. FACES OF ALL CUT AND FILL SLOPES SHALL BE PLANTED WITH A GROUND COVER INDIGENOUS TO THE AREA AND MAINTAINED AGAINST EROSION. 21. EAR7HWORK QUANTITIES ARE BASED ON THE FREE FLOATING CONCRETE SLAB IN GRADE PER THE SOILS REPORT (NOTE 1). QUANTITIES FOR EARTHWORK CUT - CU. YD. 51, 785 FILL - CU. YD. 25,120 SPOIL/STOCKPILE - CU. YD. 1J,719 SHRINKAGE FACTOR OF 25% WAS USED EXPORT TO BE USED IN OTHER PHASES. QUANTITIES ARE FOR GRADING PERMIT ONLY. THE ENGINEER MAKES NO WARRANTY OF THE ANTICIPATED SHRINKAGE FACTOR. IN CASE OF EXCESS OR SHORTAGE, THE CONTRACTOR SHALL CONTACT THE ENGINEER SO THAT ADJUSTMENTS MAY BE MADE THE CONTRACTOR SHALL NOT USE THESE QUANTITIES TO BASE HIS BID ON. 22. BUILDING PAD PREPARATION - REFER TO SOILS REPORT (NOTE 1). 23. PA VEMEN T PREPARA TION - REFER TO SOILS REPORT (NOTE 1). 24. AN OSHA PERMIT IS REQUIRED WHEN WORKERS MUST ENTER TRENCHES OR EXCA VA TION FIVE (5) FEET OR DEEPER. 25. SUFFICIENT ACCESS TO THE EXIS77NG AND ABANDONED WELLS SHALL BE MAINTAINED IN ORDER FOR THE DIVISION OF GAS, OILS AND GEOTHERMAL RESOURCES (DI VISION) TO INVESTIGATE THE CONDI TION OF THE WELL HEADS AND CHECK FOR LEAKAGE. IF ANY REABANDONMENTS ARE REQUIRED, THE DIVISION SHALL FURNISH THE NECESSARY SPECIFICATIONS TO THE PROPERTY OWNER. 26. AT THE INITIAL CONSTRUCTION MEETING PRIOR TO GRADING ACTIVITIES ALL CONSTRUCTION WORKERS MUST BE INFORMED AS TO THE SYMPTOMS OF VALLEY FEVER. 27. THE CONTRACTOR SHALL COMPLY WITH THE GUIDE FOR ASSESSING AND MITIGATING AIR QUALITY IMPACTS (GAMAQI), JANUARY 10, 2002 REVISION, GUIDANCE DOCUMENT WHICH LISTS THE FOLLOWING MEASURES AS APPROVED AND RECOMMENDED FOR CONSTRUCTION AC77077ES WITHIN THE SAN JOAQUIN VALLEY AS FOLLOWS: A. MAINTAIN ALL CONSTRUCTION EQUIPMENT AS RECOMMENDED BY MANUFACTURER MANUALS B. SHUT DOWN EQUIPMENT WHEN NOT IN USE FOR EXTENDED PERIODS OF TIME C. USE ELECTRIC EQUIPMENT FOR CONSTRUCTION WHENEVER POSSIBLE IN LIEU OF DIESEL OR GASOLINE POWERED EQUIPMENT. D. CURTAIL USE OF HIGH -EMITTING CONSTRUCTION EQUIPMENT DURING PERIODS OF HIGH OR EXCESSIVE AMBIENT POLLUTANT CONCENTRATIONS SUCH AS "SPARE -THE -AIR" DAYS AS DECLARED BY THE SJVAPCD. E. PRIOR TO S/7E GRADING, SUBDIVIDER SHALL HAVE A PRECONSTRUCTION SURVEY CONDUCTED TO SEARCH FOR NATIVE KIT FOX DENS SURVEY SHALL BE SUBMITTED TO THE PLANNING DIRECTOR FOR REVIEW PRIOR TO GRADING. THE APPLICANT MAY BE SUBJECT TO SPECIAL NO77F7CATION REQUIREMENTS TO AGENCIES PRIOR TO GRADING LAND CONTAINING DENS. 29. PRIOR TO THE APPROVAL OF A GRADING PLAN, SUBDIVIDER IS SUBJECT TO THE MIGRATORY BIRD TREATY ACT. INCLUDING THE SWANSON HAWK DURING THE RAPTOR NES77NG SEASON (FEBRUARY THROUGH SEPTEMBER). IF CONSTRUCRON IS TO OCCUR DURING RAPTOR NESTING/BREEDING SEASON (FEBRUARY THROUGH SEPTEMBER) NO CONSTRUCTION AC77WTY SHALL TAKE PLACE WITHIN 500 FEET OF A AC 77 VE NEST. THE FISH & WILDLIFE SERVICE (USFWS) AND CALIFORNIA DEPARTMENT OF FISH & GAME (CDFG) SHOULD BE NOTIFIED OF PREVIOUSLY UNREPORTED PROTECTED SPECIES. ANY UNAN77CIPATED TAKE OF PR07ECTED WILDLIFE SHALL BE REPORTED IMMEDIA TEL Y TO THE CDFG AND USFWS. 31. IF ARCHEOLOGICAL OR PALEONTOLOGICAL RESOURCES ARE DISCOVERED DURING EXCAVATION AND GRADING ACTIVITIES ON-SITE THE CONTRACTOR SHALL STOP ALL WORK AND THE DEVELOPER SHALL RETAIN A QUALIFIED ARCHEOLOGIST TO EVALUATE THE SIGNIFICANCE OF THE FINDING AND APPROPRIATE COURSE OF ACTION. SALVAGE OPERATION REQUIREMENTS IN APPENDIX K OF THE CALIFORNIA ENVIRONMENTAL QUALITY ACT GUIDELINES SHALL BE FOLLOWED AND THE TREATWENT OF DISCOVERED NATIVE AMERICAN REMAINS SHALL COMPLY WITH STATE CODES REGULATIONS OF THE AMERICAN HERITAGE COMMISSION. 32. SHOULD HUMAN REMAINS BE DISCOVERED AT ANY TIME ON ANY PORTION OF THE PROJECT, WORK SHALL HALT AND THE CORONER BE NOTIFIED IMMEDIATELY (SECTION 7050.5 OF THE HEALTH AND SAFETY CODE). IN THE ABSENCE OF AN ARCHAEOLOGICAL MONITOR, A QUALIFIED ARCHAEOLOGIST AND THE LOCAL NATIVE AMERICAN COMMUNITY SHALL ALSO BE NOTIFIED. 33 THE CONTRACTOR SHALL COMPLY WITH SAN JOAQUIN VALLEY AIR POLLUTION CONTROL DISTRICT (SJVAPCD) REGULATION VU/, DUST CONTROL MEASURES TO ENSURE COMPLIANCE SPECIFICALLY DURING GRADING AND CONSTRUCTION PHASES. THE MITIGATION MEASURES TO BE TAKEN ARE AS FOLLOWS: A. WATER EXPOSED SURFACES (SOIL) THREE- 77MES/DAY. B. WATER ALL HAUL ROADS AT A MINIMUM OF THREE TIMES PER DAY. IF THE PROJECT IS SUBJECT TO THE PROVISIONS OF THE NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM (NPDES), A NOTICE OF INTENT- (NOI) TO COMPLY WITH THE TERMS OF THE GENERAL PERMIT TO DISCHARGE STORM WATER ASSOCIATED WITH CONSTRUCTION ACTIVITY (WO ORDER NO. R5-2013-0153) MUST BE FILED WITH STATE WATER RESOURCES CONTROL BOARD /N SACRAMENTO BEFORE THE BEGINNING OF ANY CONSTRUCTION ACTIVITY. COMPLIANCE WITH THE GENERAL PERMIT REQUIRES THAT A STORM WATER POLLUTION PLAN (SWPPP) BE PREPARED, CONTINUOUSLY CARRIED OUT, AND ALWAYS BE AVAILABLE FOR PUBLIC INSPECTION DURING NORMAL CONSTRUCTION HOURS. 36. AN OPEN STREET PERMIT SHALL BE OBTAINED FROM THE CITY OF BAKERSFIELD PUBLIC WORKS DEPARTMENT FOR ANY WORK PERFORMED WITHIN EXISTING ACCEPTED STREET RIGHT OF WAY. UNLESS SECURED BY A SUBDIVISION AGREEMENT, SECURITY BASED ON AN APPROVED ENGINEER'S ESTIMATE FOR THE WORK PERFORMED WITHIN THE RIGHT OF WAY AND INSURANCE AS REQUIRED SHALL BE PROVIDED PRIOR TO ISSUANCE OF A PERMIT. 37. ALL ONSITE UNPAVED ROADS AND OFFSITE UNPAVED ACCESS ROADS SHALL BE EFFECTIVELY STABILIZED OF DUST EMISSIONS USING WATER OR CHEMICAL STABILIZER/SUPPRESANT. 38. PAD IS GRADED FLAT AT ROUGH GRADING FINISHED GRADES WILL INCORPORATE A 1% SWALE THAT WILL DIRECT DRAINAGE AWAY FROM THE STRUCTURE AND TO THE STREET SEE TYPICAL LOT SWALE GRADING ON SHEET 1. 39. WASH OFF CONSTRUCTION AND HAUL TRUCKS TO MINIMIZE THE REMOVAL OF MUD AND DIRT FROM THE PROJECT SITE. 40. DUST CONTROL CONTRACTOR SHALL OBTAIN A COPY OF THE DUST CONTROL PLAN (DCP) FOR THIS PROJECT FROM THE DEVELOPER, AND SHALL KEEP A COPY ON SITE AT ALL TIMES. FOR THE DURAT70N OF CONTRACTOR'S OCCUPANCY OF THE SITE CONTRACTOR IS REQUIRED TO COMPLY WITH THE PROVISIONS OF SAID PLAN. 41. STORM WATER POLLU77ON PREVENTION CONTRACTOR SHALL OBTAIN A COPY OF THE STORM WATER POLLUTION PREVENTION PLAN (SWPPP) FOR THIS PROJECT FROM THE DEVELOPER, AND SHALL KEEP A COPY ON S17E AT ALL TIMES.
