This endorsement modifies insurance provided under the following:
COMMERCIAL GENERAL LIABILITY COVERAGE PART
A. Section II — Who Is An Insured is amended to include as an insured any person
or organization for whom you are performing operations when you and such person
or organization have agreed in writing in a contract or agreement that such person or
organization be added as an additional insured on your policy. Such person or
organization is an additional insured only with respect to liability arising out of your
ongoing operations performed for that insured. A person's or organization's status as
an insured under this endorsement ends when your operations for that insured are
completed.
This insurance applies on a primary basis if that is required by the written contract,
written agreement or permit.
B. With respect to the insurance afforded to these additional insureds, the following
additional exclusions apply:
2. Exclusions
This insurance does not apply to:
a. "Bodily injury ", "property damage" or "personal and advertising injury" arising
out of the rendering of, or the failure to render, any professional architectural,
engineering or surveying services, including:
(1) The preparing, approving, or failing to prepare or approve, maps, shop
drawings, opinions, reports, surveys, field orders, change orders or
drawings and specifications; and
(2) Supervisory, inspection, architectural or engineering activities.
b. "Bodily injury" or "property damage" occurring after:
(1) All work, including materials, parts or equipment furnished in connection
with such work, on the project (other than service, maintenance or
repairs) to be performed by or on behalf of the additional insured(s) at the
site of the covered operations has been completed; or
(2) That portion of "your work" out of which the injury or damage arises has
been put to its intended use by any person or organization other than
another contractor or subcontractor engaged in performing operations for
a principal as a part of the same project.

Applicant agrees that if this application is granted, applicant shall indemnify, defend and hold harmless the City, its
officers agents and employees against any and all liability, claims, actions, causes of action or demands, whatsoever
against them, or any of them, before administrative, quasi - judicial, or judicial tribunals of any kind whatsoever, arising out
of, connected with, or caused by applicant's placement, erection, use (by applicant or any other person or entity) or
maintenance of said encroachment. The applicant further agrees to maintain the aforesaid encroachment during the life
of said encroachment or until such time that this permit is revoked.
Applicant further agrees that upon the expiration of the permit for which this application is made, if granted or upon the
revocation thereof by the City engineer. applicant will at his own cost and expense remove the same from the public
property or right of way where thg same is located, and restored said public property or right of way to the condition as
nearly as that in which it was before the placing, erection, maintenance or existence of said encroachment.
Applicant further agrees to obtain and keep all liability Insurance required by the City Engineer in full force and effect for
however long the encroachment remains. Applicant shall furnish the City Risk Manager with a Certificate of Insurance
evidencing sufficient coverage for bodily injury or property damage liability or both and required endorsements evidenc-
