California State Assembly Bill 2286 requires Hazardous Material Inventory and Business Plans to be
accessible to the State in an electronic form. The State of California administers the program called the
California Environmental Reporting System (CERS) which is the hazardous materials database. If you are
not able to enter any data into the CERS system for lack of computer or compatible operating system, this
office may be able to assist you.
Whereas the Bakersfield Fire Department (CUPA), enters required data into CERS on your behalf the
facility must approve and the CUPA to do so and must maintain a record of the approval on site and in
CERS.

Upon execution this form I hereby unconditionally release, waive and discharge my right, whether by
contract or under operation of law to file cause of action(s) or claim(s) which I may have against the
Bakersfield Fire Department now or in the future related to the California Environmental Reporting
System.
I hereby assume any and all risk of loss, liability, damage or costs that may incur arising out or in
connection to my acts, errors and/or omissions supplied to the Bakersfield Fire Department.
I fully understand the terms set forth in this form, and I hereby waive my rights freely and voluntarily
without any inducement, assurance, or guarantee being made to me to the fullest extent allowed by law.
