HAZARDOUS MATERIALS COMPLAINT REPORT

Location: 6501 White Lane- Valvoline
Complaint Description: Illegal disposal of oily water into a
bathroom sink.
Persons Involved: Michael Sanders
Narrative of Investigation: Upon arrival to the location I made contact with store manager Chris Marchione and explained to him
the reason for my inspection. He informed me that the store does mop above grade level which during the inspection I found no oil stains or residue. He also stated that they mop in the
underground area which occasionally gets oil on the ground but prior to mopping a solvent powder is used to clean any oil off the ground to ensure no contamination. I inspected the mop
bucket, mop head and sink area where the complaint stated the water was being dumped and no oil residue could be found. I then took the opportunity to suggest to the manager other ways
to clean the below grade area so that way no water has to be used or disposed of. The store Manager Chris said they would change the policy on mopping downstairs and I have since spoken
to corporate about the situation and the plan of action to prevent any such complaint arising in the future.
Follow-up Required: None required however I will follow up with the site
within the next 30 days to ensure that the new process for cleaning the floors in the underground area are being followed.
