Re: PetSmart Fire Alarm Monitoring Cancellation Notice
Attention: Fire Official /Life Safety Code Enforcement
Effective 12/13/2019 , PetSmart has cancelled the fire alarm monitoring agreement with F.E. Moran
Security Solutions. This letter serves as notice that F.E. Moran Security Solutions has Cancelled monitoring for the
fire alarm /life safety system for the location listed below:

As a licensed Security Company, it is our obligation to notify you of any potential issues. Based on our information
we are not confident that your life safety system was tested per NFPA 14.4.2.4 or record of completion was
submitted to you.
14.4.2.4 When changes are made to site - specific software, thefollowing shall apply:
(I) All functions known to be affected by the change, or identified by a means that indicates changes, shall be 100
percent tested.
(2) in addition, 10 percent of initiating devices that are not directly affected by the change, up to a maximum of 50
devices, also shall be tested and correct system operation shall be verified.
(3) A revised record of completion in accordance with 7.5.6 shall be prepared to reflect these changes.
This is by no means an allegation of any proven wrongdoing. As the AHJ, we trust that you will thoroughly
investigate the situation.
All required State and local alarm company licensing, system registrations, monitoring permits, communication
troubles and any /all UL certifications, runner services and general services associated with monitoring the fire
alarm /life safety system for the above referenced location, in your area, will no longer be provided by F.E. Moran
Security Solutions. Please remove our name from any /all associated or required qualifications for the required
monitoring of the fire alarm /life safety system for the above location.
If you have any questions regarding required State and local alarm company licensing, system registrations,
monitoring permits, communication troubles and any /all UL Certifications, runner services and general services
associated with monitoring the fire alarm /life safety system for the above referenced location, you will need to
contact PetSmart. I am not entirely sure of the PetSmart point of contact, but Matt Lincoln VP of Loss prevention
(623- 238 -5592, mlincolnPoetsmart.com) has been a valuable resource for us.
