RE: Provide Vacuum Truck and Disposal Services
Dear Mr. Ayala
Patriot Environmental Service (PES) is pleased to submit this quotation in accordance with information provided by Customer to provide a 120BBL
Vacuum Truck in Bakersfield, California.
Scope of Work
Mobilize Truck and Operator
Pump out, transport, and dispose of waste to an approve disposal facility.
Equipment, Personnel and Supplies
2 – 8-hour days $1,772.60
1 day to perform job, 2nd day to dipose of the waste at World Oil in Compton CA.
Disposal
$1.03 a gallon 1500 gallons total $1545.00 (additional charges per gallon if exceeds 1500 gallons)
$4.16 solid surcharge (any solids are found inside tank)
$660.00 tanker wash out fee (per tanker truck)
Rate Stipulations
• Pricing based on working Monday through Friday during normal operating hours with 24-hour notice provided or as regularly scheduled.
• Over time rate applies to over 8 hours & up to 12 hours on any given day.
• Premium time rate applies to all hours after first 12 hours in a given day.
• Proposal excludes weekends & holidays (rates provided upon request).
• Rates include transit time to and from Patriot office to the work site.
• Energy, Environmental, Insurance Surcharge 12.9 % Fee.
Clarifications
• Patriot will supply all personnel, equipment and materials to perform work.
• Customer will be responsible for additional charges due to any delays out of Patriot’s control.

THIS PROPOSAL/ BID/ ESTIMATE ARE BASED ON THE FOLLOWING QUALIFICATIONS AND EXCEPTIONS:
• Client will provide right of entry to the premises and clear access to the work areas at the time(s) and location(s) specified.
• Site conditions will not require more than Level “C” personal protective equipment. Should monitoring determine that site conditions
warrant more stringent safety precautions, Patriot will implement a higher degree of safety and notify the client concerning changes
and price adjustment provisions.
• This proposal pricing is good for 30 days from the proposal date.
• Davis-Bacon Labor Law (prevailing wage rate regulations) will not be in effect or enforced.
• Scopes of Work changes that may be required by regulatory agencies are not covered in this proposal. Additional work required will
be performed under conditions agreed upon utilizing an Extra Work Authorization/Change Order form, or by other written direction
from the client.
• This proposal is designed to meet the minimum design specifications that current regulations require. Should regulatory requirements
change, or should additional work be deemed necessary by regulatory authorities, the required work will be reviewed with the client
and billed as a contract extra.
• Patriot (Contractor) will be excused for any delay in completion of the project caused by Acts of God, acts of the client or the clients
agent, weather conditions, health and safety concerns, labor disputes, acts of public utilities, regulatory agencies, additional work
beyond the scope of the proposal, failure of the client to make payments promptly, or other contingencies unforeseen by Contractor
and beyond the reasonable control of Contractor. If delays caused by the above mentioned factors do occur, performance period
extension(s) or price adjustments equal to the delay time(s) shall be granted to Contractor, without liability to the client, for liquidated
damages due to that delay.
• The offer is valid for 30 days from the date of this letter. Any additional work beyond the scope specified in this proposal will be
negotiated and approved prior to execution.
