CLASS V INJECTION WELL 3011 Antonino Avenue Bakersfield, California 93308 1.0 INTRODUCTION This report describes the completion of subsurface soil sampling activities and laboratory analyses as required by the Kern County Environmental Health Services Department (County) at the former Class V injection well operated by M/D TOTCO, Inc. The former well was located just West of the rear of the M/D TOTCO, Inc. office and shop building at 3011 Antonino Avenue in Bakersfield (see Figure 1). Mr. Jeff Campbell, Environmental Consultant, served as project manager and site safety officer. Soil samples were collected under the supervision of and described by Mr. Dayne L. Frary, a California Registered Geologist. Ms. Flora Darling, Hazardous Materials Specialist, served as the County's on-site representative. 2.0 BACKGROUND Mr. Campbell's report, MID TOTCO. Inc. Dry_ Well Characterization and Site Remediation, provides details of the former well, clarifier, and piping system location and construction, site history, geology, hydrogeology, and all activities performed in the initial portion of the project. The initial report was dated February 4, 1993. The results of the primary soil sampling and laboratory analyses are also included in the initial report. The surface elevation of the site is approximately 400 feet above mean sea level. The depth to the uppermost water table underlying the site is believed to be betWeen 35 and 50 feet below surface grade (flosg). The water table was not reached or intercepted during borehole drilling operations. The property had been leased to M/D TOTCO, Inc. for some 14 years. The company utilized the well for waste-water collection and elimination by drainage through a gravel pack (see Figure 2). The waste water was generated as rinsate by the cleaning of oil well drilling tools with a high-pressure washer and by use of a wash basin inside the shop. Rinsate was piped by gravity to a clarifier, thence to the concrete, gravel-lined well. The total depth of the well at the base of the gravel Pack was approximately 17Jbsg. The well was altered for abandonment during Mr. Campbell's field investigation in January, I993. All were transported to VenVirotek, a licensed disposal facility in Arvin, California. The clarifier was abandoned by filling it with concrete to surface grade. The former well excavation was backfilled with clean, off-site, borrow sand. The County subsequently reqUired that additional sampling be undertaken to demonstrate, that uncontaminated native soil existed a minimum of five (5) feet below the last reported depth of excavated (removed) contaminated soil. A workplan was prepared by Mr. Frary on February 19, 1993 detailing the subsurface soil sample collection procedures and required laboratory analyses to be performed on each sample. Mr. Campbell prepared the site health and safety plan which was submitted with ,the workplan to the County. The workplan is included as Appendix A. 3.0 CONDENSED PROTOCOLS 3.1 Borehole Drilling Melton Drilling Company (Melton Drilling), under'the supervision of Mr. Frary, was utilized to drill the confirmation soil boring at the M/D TOTCO, Inc. site. Melton Drilling operated a truck-mounted, B57 drilling rig utilizing a 6-inch diameter, continuous-flight, hollow-stem auger to advance the boring. All boring equipment was steam-cleaned prior to arrival on-site. The boring was placed directly over the original Class V injection well location as shown on the Site Map (see Figure 2). Descriptions of the penetrated sediments were recorded on the boring log (see Table 1). All observed changes were noted by the geologist, and the soil types were described using the Unified Soil Classification System (see Appendix B). 3.2 Soil Sampling A 2.5-inch diameter by 18-inch length "undisturbed sample" was collected (or attempted) at 22, 27, and 32 fosg using a split-spoon sampler lined with three, 6-inch long, brass sample rings. The sampler was driven ahead of the auger bit by a 140-lb., down-hole hammer. The soil contained in the upper and middle sample rings was described, and the descriptions were recorded on the Boring Log (see Table 1). The bottom sample, when recovered, is normally selected for the laboratory analyses. The ends are sealed with Teflon or aluminum foil, capped, and wrapped with duct tape. The sample is then placed in an ice chest chilled with cracked ice (or Blue Ice) to 4 degrees Centigrade to maintain chemical integrity for transport to the laboratory. The purpose of the Soil Sampling Protocol is to ensure the accuracy of the results of 04-93 2 laboratory analyses. This is accomplished in part by the use of steam-cleaned drilling' equipment. In addition, the split-spoon samplers and brass sample rings are washed in a solution of trisodium phosphate (TSP), rinsed in tap water, and then final-rinsed in distilled water prior to drilling and sample retrieval. The sample identifications (IDs) are recorded on a chain-of-custody document. The chain-of-custody remains with the samples through transport and delivery to a California Department of Health Services (DoHS)-certified laboratory for analysis. At the laboratory the sample count, IDs, and requested analyses are verified, the documentation is signed, and a copy is retUrned to the responsible consultant. 3.3 Borehole Abandonment After the boring is drilled and sampled, it is properly abandoned to the surface using hydrated bentonite clay chips and topped with clean, off-site borrow soil. The surface is level-patched with asphalt or level-sealed with concrete, depending on composition. 4.0 FIELD INVESTIGATION 4.1 Soil Boring On March I I, 1993, Melton Drilling, under the supervision of Mr. Frary, drilled one (1), six-inch diameter soil boring through the center of the tbrmer Class V injection well for the purpose of confirmation soil sampling. The boring was drilled to a total depth of 33 jbsg. Samples were recovered at 22, 27, and 32 fosg. Following sample collection, the boring was abandoned from 33 to 22 fl~sg with hydrated bentonite chips, and from 22 Jbsg to surface grade with the clean, borrow, backfill sand. The surface Was repaired with asphalt cold-patch, restoring it to near original condition. 4.2 Soil Description The soils encountered during the investigation consisted of fluvial (river-deposited) sands which were generally fine- to medium-grained, quartzitic, very friable, and somewhat cobbly between 28 and 33 Jbsg. No hydrocarbon stains or odors were observed or noted in the three (3) split-spoon samples or in the cuttings. Detailed descriptions of these sands are provided on the Boring Log (see Table 1). 5.0 LABORATORY ANALYSES AND RESULTS 5.1 Chain-of-Custody and Transport A chain-of-custody document was prepared listing the three (3) soil samples and the JEFF CAMPBELL ENVIRONMENTAL CONSULTING BORING LOC. April 1993 required analyses to be performed by Zalco Laboratories, Inc. (Zalco) prior to transport. The samples were chilled to 4 degrees ~Centigrade in an ice chest filled with cracked ice and were immediately transported and delivered to Zalco on March 11, 1993. The chain-of-custody was signed at the laboratory upon sample receipt, and a copy was returned to Mr. Campbell. It is included as Appendix C. 5.2 Laboratory Analyses All 3 soil samples were analyzed at Zalco Laboratories in Bakersfield, California for the following constituents: 1) Total Petroleum Hydrocarbons as Gasoline by Dolls LUFT Method (EPA 8015-mod. equivalent) 2) Total Petroleum Hydrocarbons as Diesel by Dolls LUFT Method (EPA 8015-mod. equivalent) 3) Aromatic Volatile Organics (Benzene, Toluene, Xylenes, and Ethylbenzene) by EPA Method 8020 4) Volatile Organics by EPA Method 8240 5) Semivolafile Organics by EPA Method 8270 6) CAM Metals: Barium, Cadmium, Copper, and Lead by EPA Method 6010. 5.3 Analytical Results Zalco completed the laboratory analyses and reported the results on March 23 and 30, 1993. For all hydrocarbon/organic analyses listed in section 5.2 of this report, the results for the three (3) samples were nondetectable (ND) for each constituent. Only naturally-occurring background levels of the CAM metals barium and copper were reported in the 3 samples. Barium was reported at 34 mg/kg in the sample collected at 22 flosg, at 39 mg/kg in the sample collected at 27 fosg, and at 31 mg/kg in the sample collected at 32 fosg. The Soluble Threshold Limit Concentration (STLC) for barium is 100 mg/l. Copper was reported at 14 mg/kg in the sample collected at 22Jbsg, at 13 mg/kg in the sample collected at 27fosg, and at 21 mg/kg in the.sample collected at 32 fosg. The STLC for copper is 25 mg/l. Only the sample collected at '329'bsg exhibited a background level of cadmium at 0.7 mg/kg, which is below the STLC of 1.0 mg/l.. It was not detected in the overlying 22- and 27-Jbsg samples. Lead was not detected in any of the samples. The Summary of Laboratory Results is included as Table 2. April 1993 included as Appendix D. 6.0 CONCLUSIONS Based on field observations and laboratory analytical data, it is concluded as follows: 1. As depicted on the Boring Log (Table 1), the native soil underlying the former M/D TOTCO, Inc. site and Class V injection well at a depth of at least 33 Jbsg consists of river-deposited sands which are fine- to coarse-grained, pebbly and cobbly in part, and generally free of finer clastic fractions of silt and clay. 2. No stains or odors indicating the presence of hydrocarbon or other organic contamination were noted during field screening of drill cuttings, and samples collected from depths of 22, 27, and 32 Jbsg. 3. Laboratory. results of the three soil samples .(summarized in Table 2) indicated that gasoline, diesel, aromatic volatile organics, volatile organics, and semivolatile organics were nondetectable. 4. The analyses for 4 required CAM metals indicated background levels for barium and copper in the three samples. A background level for cadmium of 0.7 mg/kg was indicated only in the sample collected at 32 Jbsg; it was nondetectable in the samples collected at 22 and 27 Jbsg. Lead was nondetectable in all three samples. 5. Ground water is believed to exist between 35 and 50Jbsg. The water table was not reached or intercepted during drilling operations. 6. The underlying sands below the bottom of the former Class V injection well (total depth of the former well = 17 Jbsg) remained uncontaminated as demonstrated by laboratory analyses of samples collected at 22, 27, and 32fosg. All overlying contaminated material was removed when the well was abandoned and remediation effected during Mr. Campbell's initial investigation, 7.0 RECOMMENDATION Based upon the information submitted in this report and the above conclusions, final closure is recommended for the former M/D TOTCO, Inc. Class V injection well site. MD-TOTCO. Inc. 04-93 5 8.0 LIMITATIONS The results contained in this report are based upon the information acquired during the initial and confirmation-sampling investigations. The conclusions are based solely on and limited 'to the following: 1. Current ground water, geologic, and hydrogeologic conditions; variations in the soil and ground water, or other factors not readily apparent may exist beyond the area explored and remediated during this investigation. Changes in ground water conditions may occur in the future due to variations in rainfall, regional water use, or other factors not apparent at the time this investigation was performed. 2. Chemical compounds targeted for laboratory analysis and results documented by Zalco Laboratories, Inc. of Bakersfield, California. 3. The consultants' professional procedures, on-site observations, and knowledge of Federal, State, and Kern County regulations. The services performed by Jeff Campbell Environmental Consulting and Dayne L. Frary, Geologist have been conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the environmental services profession currently practicing under similar conditions in Kern County. FORMER CLASS V INJECFION WELL 1.0 INTRODUCTION This workplan descnbes the project background, and drilling, sampling, and analytical activities to be' performed at the former location of the Class V injection well located at 3011 Antonino Avenue, within a portion of the northwest Bakersfield ci~ limit. The plan addresses requirements submitted to MD-TOTCO, Inc by Ms. Flora Darling, Hazardous Materials Specialist with lhe Kern Coumy Environmental Heahh SerVices Department (County). Additional work is required by ~he County to identify the vemcal extent of contamination by obtaining "clean" soil samples from at least five fee! below the last recorded contaminated sampling point in order to allow for closure of the former well. The previous phase of the investigation was performed and reported by Mr. Jeff Campbell, Environmental Consultant, and was dated February d-. 1993. 2.0 BACKGROUND Mr. Campbell's report, M-D TOTCO. Inc. D~ Well Characterization and Site ~ provides details of the former well and piping system location and construction, and all activities performed pnor to the preparation of this workplan. The property has been leased to M-D TOTCO. Inc for some I't- years. The company utilized the on-site "dry well" for waste-water collection and elimination by drainage through gravel_ The waste water was generated as rinsate by die cleaning of oil well drilling tools with a high-pressure washer and by use' of a wash basin inside the neighboring shop. Rinsate was piped by gravily to a clarifier, thence to the concrete/gravel-lined well. The total dep& of ~he well at &e base of the external gravel liner was approximately 17 feet below surface grade (fbsg). At present, the well has been altered for abandonment; the concrete liner, external gravel pack, and known contaminated soil have been removed to a depth of 22 fbsg and transported to VenVirotek, a licensed disposal facility in Arvin, California. The 1 piping has also been removed, and the clarifier is to be filled with concrete which will render it useless. The excavation was backfilled with clean, off-site borrow soil 3.0 ACFIVI~ AND PROCEDURES 3.1 Soil Boring One (1), 6-inch diameter bonng will be advanced through the center of the former well location for collection of additional samples at 22. 27, and 32 fbsg. Melton Drilling Company will provide the truck-mounted rig with a continuous-flight. hollow-stem auger which will be utilized to dnll the boring. All boring equipmen! xvill be steam-cleaned prior to arrival at the site in order to minimize the possibilily of contaminating the samples. In addition, split-spoon samplers and sample rings will be washed in a solution of trisodium phosphate (TSP). rinsed in tap water, and then final-rinsed in distilled water After the bonng has been sampled at ~2 fbsg (T.D.), it will be backfilled to 22 feet ~vith bentonite pellets and then with the clean borrow soil presently in the excavation to the surface. Any additional soil found to be contaminated will be transported under manifest to the VenVirotek disposal faCility on the scheduled work day. 3.2 Soil Sampling 2.5-inch diameter by 18-inch length "undisturbed" samples will be collected at 22, 27, and 32 fbsg using a split-spoon sampler lined with three (3), 6-inch, brass sample rings. The sampler will be driven ahead of the auger bit at the appropriate depth by a l't-O-pound, down-hole hammer It will not be necessary to collect samples in the clean import soil belween surface grade and 22 fbsg. The soils recovered in the upper and middle nngs will be descnbed by the California Registered Geologist, who will record the descriptions on the boring log according to the Unified Soil Classification System (USC, S). The same samples will be screened for initial indications of hydrocarbon contamination using a' photoionization detector (PID). The PID 'readings will also'be recorded on the 2 boring log. The lower sample will be sealed with Teflon gaskets, capped, final- sealed with duct tape, labeled, and placed in an ice chest chilled with Blue Ice to ar° Centigrade to preserve its chemical integrity'. The sample identifications and their requested laboratory analyses will be recorded on a chain-of-custody document. The chain of-custody will remain with the samples dlrough delivery to Zalco Laboratories, Inc.. a State-certified laboratory: a copy of the completed chain-of-custody will be returned to the geologist at the time of delivery. Upon completion of die analyses, the reported results will be signed and certified by the chief chemist and mailed to the geologist. 3.3 laboratory Analyses A total of three (3) soil samples will be delivered to Zalco Laboratones, Inc. in Bakersfield on die scheduled work day. The following analyses will be specified for each sample: Volatile Organics by EPA Method 82't-0 Semivolatile Organics by EPA Method 8270 TPH as gasoline by EPA Modified Method 801 S TPH as diesel by EPA Modified Method 80155 Aromatic Volatile Organics by EPA Method 8020 CAM metals: Banum, Cadmmm, Copper, and Lead ~.0 REPORT PREPARATION Upon receipt of die laboratory analyses, the geologist will prepare a detailed report of the drilling and sampling program, including all laboratory results for the samples. Additional illustrations such as revised site maps and cross-sections will be included as needed. Conclusions and recommendatiOns will be considered and reported pending die outcome of the laboratory analyses. Rinsate was piped by gravily to a clanfier, thence Io the concrele/gravel lined well. The excavation was backfilled with clean, off-site borrow soil. 3.0 ACTIVi'I'IbZS AND PROCEI)IJRES 3.1 Soil Boring One (i'), 6-inch diameter bonng will be advanced through the center of the t0rmer well location for colleclion of additional samples at 22, 27, and 32 f'bsg. Mellon Drilling Company will provide the truck-mounted ng with a continuous-flight, hollow-slem auger which will be utilized to drill the bonng. All I~)nng equipment will be steam-cleaned pnor to arrival at the site in order to minimize the possibilily of contaminaling Ihe samples. In addition, splil-slxx)n samplers and sample nngs will be washed in a solulion of Imodium phosphale (TSP), rinsed in lap wmer, and then final-nnsed in distilled water. The soils recovered in the upper and middle nngs will be descnbed by the California Registered Gcx)logisl. who will record Ihe descnptions on the bonng log according to the Unified Soil Classificanon System (USUS). The same samples Will be screened t0r initial indicalions of hydrocarbon contamination using a photoionizmion detector (PID). The PID readings will also be recorded on the boring log. The lower sample will be sealed with Teflon gaskets, capped, final- sealed with duct tape, labeled, and placed in an ice chest chilled with Blue Ice ar° Centigrade to preserve its chemical integrity. The sample identifications and their requested laboratory analy~es will be recorded on a chain-of-cuslody documenl. This health and safety plan establishes the policies and procedures which protect workers and the public from potential hazards posed by work at a typical site. J.C.E.C. considers safety the highest priority during work at a site containing potentially hazardous materials and has establish a policy of minimizing exposure which must be upheld on all projects. All project activities will be conducted in a manner that minimizes the probability of injury, accident or incident occurrence. The plan does not focus on the specific work activity planned for a specific site. It must remain flexible because of the nature of~ that require deviations from the original plan. This flexibility allows modification by the J.C.E.C. site supervisors. This plan takes into account the preliminary information currently available from discussions .with the project management team. This plan is written in accordance with SB 198 CAL / OSHA Safety Orders. Site History and Scope of Work: J.C.E.C. performs site investigations, site characterizations, site remediation and similar environmental related surveys. These sites Commonly involve borehole sampling procedures. Borehole sampling often invOlves.a borehole drilling rig, which employs the use of a hollow stem auger and a split spoon sampler. Key Personnel and Management: J.C.E.C. maintains a policy of providing its personnel, subcontractors, and authorized visitors with information and procedures in order to protect them and the adjacent community from and adverse effects that might result from the work at the job site involving potentially hazardous substances. All JCEC personnel will read and agree to comply with the health and safety procedures set forth in this manual. Visitors Will not be admitted to the site unless they have also read and agreed to these procedUres. Site Operations Supervisor: J.C.E.C. designates a Site Operations Supervisor (SOS) who implements and enforces project safety programs and procedures. The SOS will ensure that the daily safety meetings are held and will interface as required with other sire representatives. The SOS takes the following action(s) when appropriate: Orders the immediate shut down of site activities in the case of a medical emergency or unsafe practice. Ensures protective clothing and equipment are properly used, maintained and stored ~ Restricts visitors from areas of potential .eXposure to harmful substances. A safety log will be kept for all J.C.E.C. activities. This log will include daily safety meeting topics,' training given, air monitoring information, first aid administered, visits of all outside personnel and any incidents of a health and safety nature. The SOS has responsibility for implementing and enforcing the safety program and procedures. He will oversee any personnel monitoring and will decide when action levels have been reached which require more stringent protection. The SOS establishes and enforces the protective equipment to be used for various site activities and will maintain contact with the J.C.E.C. Safety Administrator. Field Project Manager: The Field Project Manager (FPM) has responsibility for all field activities and enforces safe work practices by all crew members. He watches for any ill effects on any of the crew members, especially those symptoms caused by heat stress of chemical exposure. The FPM oversees the safety of. anyone who enters the site and maintains communication with J.C.E.C. Site Operations Supervisor. Equipment Operators: Equipment operators will be responsible for the maintenance, inspection and safe operation of all equipment. Personnel Safety Responsibilities: Although the employer is responsible for providing a safe and healthful work place, each employee is responsible for his / her supervisor. The well has been identified as a wash rack connected to a drywell. This notice is being sent to YOU because you have been identified as the operator of the injection well located at the property listed above. Pursuant to Code of Federal Regulations 40 (40 CFR), Section 144.12 (a), "no owner or operator shall.., operate [an injection well].., in a manner that allows the movement of fluid containing any contaminant into underground sources of drinking water, if the presence of that contaminant may cause a violation of any primary drinking water regulation.., or may otherwise adversely affect the health of persons." This Department is aware that closure of the well and site characterization has been initiated prior to this notice. All site work must comply with the KERN COUNTY AND EPA REGION 9 GUIDELINES FOR CLOSURE OF SHAIJ.OW DISPOSAL WELLS. The booklet cites the minimum required for site work plan activities, contractor requirements, example illustrations of well designs, and sampling methods and procedures. Be advised that KCEHSD may have additional requirements not listed in the EPA closure guideline booklet. In accordance with Kern County Ordinance Code G-5541, Section 8.04.100 the Department is authorized to charge for inspections, review of repons, and. laboratory service costs incUrred in the process of obtaining compliance with any violations noted. The report is not acceptable to this Department due to several factors which have not been satisfactorily addressed. 1. The laboratory analysis of selected soil samples have demonstrated, that Barium, Copper, Cadmium, and Lead exceed drinking water standards. 'Additional soil analyses are required.. 2. Soil samples for laboratory analyses of TPH diesel, TPH gasoline, and BTEX was not collected at the lowest level of the excavation. The Environmental Protection Agency (EPA) requires a minimum of five feet of clean soil beneath the lowest part of the contaminati°n plume. Confirmation samples were not collected at the total depth of the excavation (22 feet below surf'ace grade). 3. Laboratory analyses of soil for semi volatfles (solvents) was not performed. 4. A professional geologist or cml engineer registered in'the State of California did not perform the work or sign the report as required by EPA Region 9 and Kern County Environmental Health Services Department guidelines for shallow disposal wells.
