ROAD AND 3207 ANTONINO AVENUE, BAKERSFIELD, CALIFORNIA
Dear Mr. Antonino:
This letter confirms the completion of site investigation at the above locations. With the
provision that the information to this agency was accurate and representative of existing
conditions, it is the position of this office that no further action is required at this time.
Please be advised that this letter does .not relieve you of any liability under the .Code of
Federal Regulations or Water Code for past, present, or future operations at the site.-
Further, it does not relieve you of the responsibility to clean up existing, additional, or
previously unidentified conditions at the site which cause m: threaten to cause pollution or
nuisance or otherwise pose a threat to water quality or public health.
Additionally, be advised that changes in the present or proposed use of the sites may require
further site characterization and mitigation activity. It is your responsibility to notify this
agency of any changes in report content, future contamination findings, or site usage.

It is our intention to cap the drain in the drywells and plug
the catch basins with concrete.
At the 3304 Pierce Road site, we will install'a pipe drain to
accept water from.our fce machine.
We anticipate being complete within 30 days of your acceptance.

Dear Mr. Antonino:
Your proposal for closure of the washrack at the 3304 Pierce Road facility is acceptable to -
the Department. However, you or your environmental contractor must submit to this office
-. a workPlan detailing the abandonment procedures and a time table for completion of the
abandonment of the wells at both sites in accordance with U.S. EPA Guidelines for Closure
of Class V Shallow Injection Wells.
The workplan must be submitted to this office for review and approval before any further
work begins. The workplan should be submitted to this office within thirty (30) days of
receipt of this letter. Additional use of the drainage system is not permitted.

the Class V shallow injection well at the site noted above. Our records indicate that
compliance has not been obtained. In order to avoid further legal action you must comply
with the original order dated March 1, 1993, within fourteen (14) days of receipt of this
letter.

AVENUE, BAKERSFIELD, CA
Dear Mr. Antonino:
The results of the samples retrieved March 23, 1993, by B. C. Laboratories at the location noted
above have been received by this office. Our review has determined that the hazardous waste toxicity
criteria for metals have not been exceeded in the liquid sampled pit, nor does the liquid sampled
exceed the Minimum Contaminant Levels (MCL) as published in the U.S. Environmental Protection
Agency (EPA), Drinking Water Regulations and Health Advisories of 1992.
Further soil investigations will not be required based upon the acceptable levels demonstrated by the
sample analyses of April 5, 1993. However, the existing drainage system and all lines leading to it
must be abandoned in accordance with U.S. EPA Guidelines for Closure of Class V Shallow Injection
Wells.
You or your environmental contractor must submit a workplan to this office for review and approval
before any further work begins. The workplan should be submitted to this office within thirty (30)
days of receipt of this letter. Additional use of the drainage system is not permitted.

A notice was mailed on December 11, 1992, directing you to initiate sampling and closure
activities of the Class V shallow injection well at the site noted above. Our records indicate
that compliance has not been obtained.
In an attempt to reach a mutually agreeable resolution to this matter, an admirfistrative
hearing has been scheduled. Please appear at the Kern County Environmental Health
Services Department, 2700 "M" Street, Suite 300, Bakersfield California. In the event the
date listed below is not convenient, please inform this office immediately so that another
date may be scheduled.

Dear Mr. Antonino:
The results of the samples retrieved January 21, 1993, by B C Laboratories at the location noted
above have been received by this office. Our review has determined that the hazardous waste toxicity
criteria for metals has not been exceeded in the soil sampled under the leach line, nor does the soil
sampled exceed the Minimum Contaminant Levels (MCL) as published in the U.S. Environmental
Protection Agency (EPA), Drinking Water Regulations and Health Advisories of 1992.
Further soil investigations will not be required based upon the acceptable levels demonstrated by the
sample analyses of January 27, 1992. However, the existing drainage system and all lines leading from
it must be abandoned in accordance with U.S. EPA Guidelines for Closure of Class V Shallow
Injection Wells.
You or your environmental contractor must submit details of how the existing lines will be
abandoned. The workplan should be submitted to this office within thirty (30) days of receipt of this
letter. Additional use of the drainage system is not permitted.

determined by Kern County Environmental Health Services Department to be the possible
site of a Class V shallow injection well. The well has been identified as a waste disposal
drain. This notice is being sent to you because you are the owner of the property listed
above.
Pursuant to Code of Federal Regulations 40, (40 CFR), Section 144.12 (a) provides that "no
owner or operator shalL_operate [an injection well]...in a manner that allows the-movement
of fluid containing any contaminant into underground sources of drinking water, if the
presence of that contaminant may cause a violation of any primary drinking water
regulation...or may otherwise adversely affect the health of persons."
There are indications that a waste disposal drain located to the east of the office building
discharges to the ground. You may demonstrate that the drain is not regulated under the
Class V Shallow Injection Well Program within thirty (30) days of receipt of this notice. If
the clarifier is determined to be a Class V well, characterization work must be initiated as
outlined in the enclosed copy of EPA-REGION 9 GUIDELINES FOR CLOSURE OF
SHALLOW DISPOSAL WELLS. The booklet cites the minimum required for site work
plan activities, contractor requirements, example illustrations of Well designs, and sampling
methods and procedures. Be advised that KCEHSD may have additional requirements not
