The approximately 150 compressed gas bottles which will remain out on the asphalt must be restrained and protected from physical damage until the new storage facility is complete. The temporary protective and restraint measures may include use of the cylinder transport gondolas which we discussed during our last meeting. I am currently checking to see if the transport gondolas may be considered an acceptable long term restraint arrangement. The plans for the new facility must be submitted to the. Bakersfield Building Department for permitting and review. We will conduct a follow up inspection to verify that the plan submitted on 8-28-91 has been implemented and that the remaining cylinders are secure by 10-28-91. The proposed time frame for completion of this project, 90 days, is acceptable. I have attached a handout regarding acceotable techniques for securing cylinders. The plan for the cylinder restraint and protection from vehicles must be approved by the Bureau of Fire Prevention. Please submit the plan to me at 2130 G Street and I will see that it is ~ reviewed by the Bureau of'Fire Prevention. Be sure to check with the Bakersfield Building Department to see if the proposed work requires a building permit. In October of 1990, I advised Mr. Fowkes of Linde on the Fire Code requirements for the siting~of a hydrogen storage tank on this same area 'of the Baker property. Please be aware that the installation of these racks or dock area may affect the prior plan for the hydrogen tank. The Uniform Fire Code, Section 80.301(n), also requires segregation of incompatible hazardous materials. Segregation may be accomplished by separating the materials by a distance of 20 feet or by providing a noncombustible partition extending not less than 18" above and to the sides of the material. In planning the new storage area and managing the existing-one, keep in mind the necessity to segregate oxidizers and toxics from flammables. The Fire Fi~hter:s inspection of Baker Welding in December 1990 indicated that the chlorine and anhydrous ammonia have been removed as proposed by the Risk Management and Prevention Plan. -I have not yet received an inventory revision to indicate this deletion. Plant staff ~ust be able to transmit emergency information to management for their use in evaluating the problem. Outside regulatory agencies must also be notified of the problem aa that appropriate action to protect the general public and the environment. NOTIFICATION ~J~enever there is an i~minent or actual emergency situation resulting from a fire, explosion and/or chemical release, it is the legal responsibility of the emergency coordinator (or his designee) to notify appropriate State and/or local agencies with assigned response roles if their help is needed. If the emergency coordinator determines that the facility has had a release, fire or explosion which could threaten human health or the environment outside the facility, thus requiring (in his opinion) an evacuation of local areas, he must notify appropriate local authorities. He must also be available to help appropriate officials decide whether local areas should be evacuated. The report must include the following information: A. Name and telephone number of the reporter. B. Name and address of the facility. C. Time and type of incident (fire, explosion, chemical release). D. Name and quantity of material(s) involved to the extent known. T~e extent of injuries, if any. F. The possible hazards to human health or the environment outside the facility. E~ERGENCY PROCEDURES You ~ust have a set of procedures, ~hich include instruction or ~uides to be followed by employees, supervisors, ·nd the emergency coordinator during emergencies at your facility. These procedures ere basic to · co~plete Emergency Contingency Plan. The guides and checklists can be short end simple and tailored to your facility but ~ust be in ~ritten form and available during an emergency. -6- PLANT AND CUSTOMER EMERGENCY PLAN Plant: When an emerqency* situation arises, follow the instructions as listed below: 1. Sound the evacuation alarm/horn/system. 2. Evacuate the plant and store areas immediately. 3.Shut off the "Main Power Supply" and any other pertinent equipment if necessary. 4. Report to the evacuation staging area and wait for instructions from the emergency coordinator. A head count utilizing the visitors log and employee list will be taken at this time. 5. The emergency coordinator or his designee is responsible for notifying the proper authorities - fire department, police department, and/or hospital if necessary. 6. In certain circumstances the employee should sound the Plant Emergency Alarm (PET) and then contact supervison. - 12- RESPONSIBILITIES E~ERGENCY PROCEDUI~S GUIDE EH~RSENCY COORDINATOR (EC) 1. ~henever there is an actual emergency situation the emergency coordinator (EC) must: a. Activate internal facility alarms or communication systems (vhere applicable) to notify all personnel of the danger. b.Notify appropriate atate or local response agencies if their help is needed. 2. l~henever there is a release, fire or explosion the EC ~ust immediately identify the nature of tbs problem, exact source, emount and extent of any released materials. This can be done by direct observation or use of records, manifests or material safety data sheets or by chemical analysis if necessary. 3. The EC must assess possible hazards to human health and the environment, resulting from the release, fire or explosion. Examples include toxic gases or hazardous materials running off site when control measures are used. &. If local areas may need to be evacuated, the EC is responsible for notifying local authorities. He must also be available to assist officials in the decision to evacuate. 5. The EC ~ust notify state or federal agencies if the emergency extends outside his facility. 6. During the emergency the EC ~ust take all reasonable steps to ensure that fires, explosions or releases do not spread to other hazardous materials or vastes stored at the facility. Control measures must include: stopping processes end operations, collecting end containing released hazardous materials or vastes and removing or isolating collected hazardous ~aterials. ?. Similar reporting requirements may apply under CERCLA (Comprehensive Environmental Response, Compensation and Liability Act). SARA and CERCLA require reporting when a Reportable Quantity (RQ) of a chemical is released into the air, water, or soil and extends beyond the facilities boundaries. They BOTH have a separate list of chemicals in which a large portion are in common. In addition to the two lists, CERCLA requires reporting any oil spills which produce a sheen on any water's surface. Initial Action Plan - Receipt of Bomb Threat 1. Handling the call a. Most likely persons to receive a bomb threat call are: Telephone Operator during normal working hours; Operator or Shift Foreman during hours other than normal working hours; Or Plant Manager. b. Keep the caller on the line as long as possible. Ask the Caller to repeat the message. Record every word spoken by the person making the call. c. If the caller does not indicate the location of the bomb or the time of possible detonation, the person receiving the call should ask the caller to provide this information. d. It may be advisable to inform the caller that the building is occupied and the detonation of a bomb could result in death or serious injury to many innocent people. e. Pay particular attention for any strange or peculiar back- ground noises, such as: motors running, background music and the type of music, and any other noises which might give even a remote clue as to the place from which the call is being made. 2. Notifying proper authority. a. Notify your supervisor immediately after receipt of message. He or she will notify the decision making team. This team will immediately gather in the Plant Manager's office with the person making the call. (If person receiving the call~ was someone other than a team number.} 3. The items for consideration by the decision making team. a. If the caller appears to be just talking and is relaying the message as it occurs to him, i.e., "off the top of his head", the chances are fairly good that it is just a crank call. b. If the caller makes two or more calls in succession, carefully compare each of the calls, one with the other. If the language or words used are different, the threat may be considered "off the top of the head type". c. If the message appears to be carefully worded and sounds as if it is being read as a previously prepared statement, the chances are good that this is the real thing. d. In general, any response to a bomb threat would fall into one of three possible categories. The decision making team will determine what reaction would be best. These three could be labeled: A. Hoax probability - search with minimum of disruption. B. Localized threat possibility - partial evac- uation of people indicated, as warehouse. C. Major threat - complete evacuation. Search importance secondary. OPTION PLAN i (Probable Hoax) 1. Notify public authorities. Their advise and assistance in making the most effective use of your search capabilities is important. 2. There are several levels of search depths possible from a "cursory look - 21 - through" to a "thorough all the way" to a "sweep". The most probable for our plant would be to have each department look for anything suspicious or out of place. Look for something that is not normally there such as a lunch box by a compressor. It would be nearly impossible for the average organization to conduct a plant search that could assure 100% security. OPTION' PLAN II (Localized Threat) 1. Partial evacuation and search could come into play under certain conditions. For example, if the caller mentioned a specific section that could be isolated like the warehouse, a quick search could be completed without the plant area being interrupted. OPTION PLAN III (Major Threat) 1. Total evacuation would be indicated if the management team feels an immediate and urgent threat is present. B. Procedure for Handling a Confirmed Bomb Threat .. This procedure is to be placed into action if the decision making team reaches the conclusion that the plant has a real and serious bomb threat. 1. Note: Army regulation's require some local civil authority be notified. They will then request a bomb disposal unit to take care of the problem. 2. Sound fire alarm to begin evacuation plan. If different evacuation routes or assembly points are decided upon, the member of the decision making team that sounds the alarm will notify personnel involved. 3. Operating units may or may ~ot be evacuated and/or shutdown. The decision making team will determine whether or not to - 22 - " evacuate. In the event evacuation is decided upon, the assembly point for operating personnel will be announced. C. Procedure for Handling a Live or Suspected Live Bomb 1. Shut down all conventional radio transmission in the plant. 2. Notify all mobile radio units to remain silent until further notice. SJ Use of "Walkie - Talkie" radios may be permitted BUT at distances of 300 feet or greater from the bomb location. 4. Shut down the building or the unit involved if requested bX Decision Making Team. a. Evacuate quickly, but orderly. b. Open all windows and doors of the building. 5. Erect barricades around the device using: a. Bags of vinylite b. Bags of sand ¢. Bales of hay .. d. Do not use wood, metal or solid materials that might blow into fragments. 6. When erecting barricades do not disturb the device. DO NOT UNDER ANY CIRCUMSTANCES TOUCH OR DISTURB OR ATTEMPT TO DISARM THE DEVICE!! D. Bomb Search Team 1.It will be the decision of the management team to request volunteers for searching or utilize the local civil authorities as a search team. E.General Recommendations for Searching for a Bomb 1. Searching buildings and non-operating areas. a. Start from the outside of the building and work in. b. Start from the bottom of the building and work up. c. Start with easily accessible closets, stairwells, elevator shafts, nooks and crannies inside of the building. d. Then check room by room again using the outside-in procedure. 1. Look for strange or unusual objects - check desks file cabinets, etc. 2. Look over floor area very well. 3. Stand near center of room and make sweep: a. Floor to eye level. b. Eye level to ceiling. 4. Look closely for recently disturbed items and fixtures in the room. 5. Examine the furniture for possible hidden or concealed items. 6. Check ventilator ducts-and cold air returns. e. The search must be orderly and well planned s as to give the best possible assurance that no suspicious objects are overlooked. f. Place an identifying mark on edge of lower left hand corner of the door. This mark indicates that the room has been searched and is OK. a. If caller gave the time that the bomb is to go off, conduct the search so that: 1. Approximately 30 minutes before and 60 minutes after the announced time, all personnel have been evacuated. b. Search may be resumed after that time if deemed necessary; - 24 - 5.2 PROCEDURES 5.2.1 In the event a bomb threat is received: 5.2.1.1 Listen. (See 5.3.2. Harger Dear Sir: Thank you for your recent inspection of our Pierce Road location. I would like to discuss the requirement for securing all compressed gas cylinders. We are very aware of the danger of falling cylinders and have, through the direction of our cylinder distributing plant in Torrance, developed a way of securing cylinders without pipes and chains. We manufacture oxygen so we have a constant movement of cylinders. We distribute and receive 400 to 600 cylinders daily. Chaining and unchaining that many cylinders during the course of a day's business would be most difficult, impractical and inefficient. Our method of security is thru interlocking the cylinders by off-setting or what we call "staggering" them. (See illustration).
