Dear Steve, We have chosen Jorgensen & Co. to provide the automatic fire suppression system for our fuel vaults. The system will be Ansul Ind-X dry chemical using Ansul Automan IIC mechanical actuation control panel and fusible link detection. The actuation devices will be located at least 20' away from the vaults inside a weather proof enclosure with audio and visual alarms. Also, per your request I have enclosed a copy of our Confined Space Program and copies of our Certification of Training for: COnfined spaces, CPR & First Aid. I think we will be ready for your inspection of our fuel system in about two weeks. Either John or I will call to schedule a time. If there is any thing else you will need please let me know. Davies Oil Company Confined Space Program Proposed 1. Introduction These procedUres prescribe the minimum safety standards for entry into confined spaces. Definitions of some of the important terms: Confined Space - An enclosed space where existing ventilation is insufficient to remove dangerous air contamination or oxygen deficiency which may exist or develop and access or exit for the removal ora disabled employee is difficult due to the location or size of the openings. Potential Hazards - Petroleum distillate fuel (gas and diesel) may present a toxic, flammable, or oxygen deficient atmosphere. Exposure limit for total product is 100 ppm for an 8 hour exposure. Oxygen level must be greater than 19.5% by volume. 2. Training Prior to entry into or work in confined spaces, all the affected employees will be trained in and provided a copy of these operating procedures. Training will be done each time there is a new hazard, change in procedures, etc., but not less than annually. 3.. Pre-entry Requirements All vaults will be considered permit required confined spaces until testing has been completed. The air within the space will be tested for dangerous air contamination or oxygen deficiency by a qualified person and a written record of such testing will be maintained at the job site for the duration of the work. If air testing demonstrates that the dangerous air contamination or oxygen deficiency does not exist, entry into and work within the space may proceed, provided additional air testing is conducted to ensure air contamination does not occur. Test results will be documented on the pemfit. All doors will be open to ensure adequate ventilation and will be maintained while work is in progress. The confined space pre-entry permit (and this procedure) will be at the site and appropriate sections checked off to ensure safe entry and work. If circumstances cause an interruption of work or a change in conditions form which the entry permit was issued, a new permit must be completed. 4. Confined Space Entry If there are no non-atmospheric hazards present and if the pre-entry tests show there is not dangerous air contamination and/or oxygen deficiency within the space and there is no reason to believe that any is likely to develop, entry into and work within may proceed. Continuous testing of the atmosphere will be done. Authorized entrants will immediately leave the space when any of the conditions change and not return until a trained supervisor has tested to ensure that it is safe to enter. When an atmosphere free of dangerous air contamination or oxygen deficiency cannot be ensured or maintained through ventilation, the following procedures will be enforced: Self contained breathing apparatus will be provided and worn in accordance with our respiratory protection program. A safety harness, that suspends a person in an upright position, with and attached lifeline will be used. This lifeline will have at least a 3,000 pound breaking strength. If an entry is made through a top opening, a hoisting device will be provided to lift employees out of the space. At least one trained person (attendan0 will stand by outside of the confined space to give assistance in case of an emergency. One additional trained person (standby) will be within calling distance. The attendant will be equipped with a self contained breathing apparatus. Effective communications will be provided between attendants and those inside the confined space. At least one employee trained in CPR / First Aid will be immediately available whenever respiratory protection is required. An attendant may enter a confined space only after alerting a standby outside the confined space of the attendant's intent to enter the confined space. Box 1390 Fontana, CA 92335 Dear Mr. Doyle: VAULT EXEMPTION FROM THE UNDERGROUND STORAGE TANK (UST) LAW This letter is written i,~'response to your request to Arron Rambach of this office for clarification of the vaulted-tank exemption from tile UST law. Section 25283.5 of the Calitbrnia Health and Safety Code exempts tanks below grade in a vault from the UST law if the system meets certain conditions. These exempted 'tanks do not have to be permitted to operate by the local implementing agency (LIA) and are not subject -- to the UST storage tee administered by the State Board of Equalization. You have described a vaulted tank situation involving a UL, 142 aboveground storage tank that is raised off the floor of a concrete vault. There are no remote dispensing or remote fills. You explain that the tank operator can climb into the vault and view ali ~urfaces of the tank and piping and the floor directly beneath tile tank and piping on a daily basis for unauthorized releases. You also describe that the operator can walk along one long side ot the tank and one end or' tile tank. Althougi~ he can not walk along the back side and opposite end, he can look around corners to observe the backside and opposite end or' the tank. This vaulted tank situation meets tile exemption requirements and would not be regulated by State .............UST law as-long.as a--Written-log.documenting-the results of the daily-visual-inspections is maintained at the site and made readily available to LIA inspectors for review at any time. In addition to tile above', in order to be exempt the concrete vault must meet secondary · containment requirements. Definitions of some of the important terms: Confined Space - An enclosed space where existing ventilation is insufficient to remove dangerous air contamination or oxygen deficiency which may exist or develop and access or exit for the removal of a disabled employee is difficult due to the location or size of the openings. Potential Hazards - Petroleum distillate fuel (gas and diesel) may present a toxic, flammable, or oxygen deficient atmosphere. Exposure limit for total product is 100 ppm for an 8 hour exposure. Oxygen level must be greater than 19.5% by volume. 2. Training Prior to entry into or work in confined spaces, all the affected employees will be trained in and provided a copy of these operating procedures. Training will be done each time there is a new hazard, change in procedures, etc., but not less than annually. 3. ~ Pre-entry Requirements All vaults will be considered permit required confined spaces until testing has been completed. The air within the space will be tested for dangerous air contamination or oxygen deficiency by a qualified person and a written record of such testing will be maintained at the job site for the duration of the work. If air testing demonstrates that the dangerous air contamination or oxygen deficiency does not exist, entry into and work within the space may proceed, provided additional air testing is conducted to ensure air contamination does not occur. Test results will be documented on the permit. All doors will be open to ensure adequate ventilation and will be maintained while work is in progress. The confined space pre-entry permit (and this procedure) will be at the site and appropriate sections checked off to ensure safe entry and work.
