Sampling was conducted using a Geoprobe Model 5400 sampling unit, which is a hydraulically powered probing instrument, similar to a cone penetrometer, that advances 1-inch diameter sampling tools into the subsurface to retdeve soil and groundwater samples. Sampling was accomplished by advancing threaded alloy steel probe rods driven by a GH-40 hydraulic hammer. The three soil samples were recovered at 20, 25, and 30 feet below ground surface (bgs) from one sampling location advanced at an angle underneath an existing underground storage tank. Upon reaching each sampling depth, a soil sample was recovered using a brass liner placed inside a stainless steel sampler. The sampler was driven into the subsurface for retrieval of the undisturbed soil sample. ,Upon retrieval, the sample liner was immediately capped, taped, labeled in accordance with Environmental Protection Agency (EPA) protocols, and chilled pending transportation to BC Laboratories, Inc., a State-certified analytical laboratory, following chain-of-custody procedures. The sampled location was abandoned by backfilling with bentonite pellets. The three soil samples were analyzed for Total Petroleum Hydrocarbons as gasoline (TPHg) by EPA Method 8015, and for volatile aromatic compounds (BTEX: Benzene, toluene, ethyl benzene and total xylenes) by EPA Method 8020. None on these analytes were detected by the laboratory above the practical quantitation limits. The laboratory data are enclosed. Additionally, the three soil samples were also analyzed for Total Lead by EPA Method SW-6010. The Total Lead concentrations reported by the laboratory were: 5 rog/kg at 20 feet bgs, 3 rog/kg at 25 feet bgs, and 4.84 mg/kg at 30 feet bgs. Sampling was conducted using a Geoprobe Model 5400 sampling unit, which is a hydraulically powered probing instrument, similar to a cone penetrometer, that advances l-inch diameter sampling tools into the subsurface to retrieve soil and groundwater samples. Sampling was accomplished b~, advancing threaded alloy steel probe rods driven by a GH-40 hydraulic hammer. The three soil samples were recovered at 20, 25, and 30 feet below ground surface (bgs) from one .sampling location advanced at an angle underneath an existing underground storage tank.' Upon reaching each sampling depth, a soil sample was recovered using a brass liner placed inside a stainless steel sampler. The sampler was driven into the subsurface for retrieval of the '... .::'. undisturbed, soil sample. Upon retrieval, the sample liner was immediately Capped, taped, labeled in accordance with Environmental Protection Agency (EPA) protocols, and chilled pending.transportation to BC Laboratories, Inc., a State-certified analytical laboratory, following chain-of-custody procedures. The sampled location was abandoned by backfilling with bentonite pellets. The three soil samples were analyzed for Total Petroleum Hydrocarbons as gasoline (TPHg) by EPA Method 8015, and for volatile aromatic compounds (BTEX: Benzene, toluene, ethyl benzene and total xylenes) by EPA Method 8020. None on these analytes were detected by the laboratory above the practical quantitation limits. The laboratory data are enclosed. Additionally, the three soil samples were also analyzed for Total Lead by EPA Method SW-6010. The Total Lead concentrations reported by the laboratory were: 5 mg/kg at 20 feet bgs, 3 mg/kg at 25 feet bgs, and 4.84 mg/kg at 30 feet bgs. A Work, Health and Safety Plan was prepared, read and signed by all field personnel prior to commencement of operations. The boring was drilled using an AMS extendable hand auger capable of retaining a two inch diameter by six inch long brass sampling tube to obtain soil samples. ~t was located eight feet south of the center of the tank and was drilled to a depth of fifteen feet at a 34° angle from vertical. This equated to a depth of six feet below the base of the tank. The soil samples collected in the brass tubes were capped, taped, labeled and placed in cold storage of four degrees Celsius or less for transport to BC Laboratories, a State Certified Laboratory. Chain of Custody documentation was maintained. The auger head was cleaned prior to sampling, between sampling intervals, and between borings to minimize the possibility of cross contamination. Soil samples were collected at ten foot and fifteen foot depths below the ground that equated to two feet and six feet beneath the base of the tank. They were analyzed for the constituents listed below: Benzene, Toluene, Ethylbenzene and Xylene (EPA Test Method 503018020) Total Petroleum Hydrocarbons - Gasoline (EPA Test Method 8015M) Total Lead (EPA Test Method SW6010) The boring was backfilled with bentonite chips and wet down. Immediately after sampling, the tubes will be sealed with teflon, capped with plastic end caps, sealed with duct tape, labeled and placed in a cooler containing blue ice and kept at a temperature of four degrees Celsius or less. Chain-of-Custody forms will be completed when the sample is collected and maintained with each transfer of the samples. Soil samples will be transported to a State Certified Laboratory for analysis. The borings will be backfilled with bentonite, cuttings and a concrete cap. 4.0 HEALTH AND SAFETY PLAN 4.1 PURPOSE The purpose of this Health and Safety Plan is to provide for worker and environmental Health and Safety during performance of the Phase II Site 2 Assessment at Star Transmission located at 1222 East California Avenue, Bakersfield, California. This project will be conducted by A J Environmental, Inc. 4.2 SITE HISTORY This Phase II Site Assessment Health and Safety plan proposes a methodology to determine the presence or absence of contamination beneath the underground tank located at Star Transmission, 1222 East California Avenue, Bakersfield, California. The work being performed addresses the proposal prepared November 11, 1997. The date of the tank installation is unknown. Task 1: Preparation of a Phase II Site Assessment Work Plan and Health and Safety Plan. Task 2: This program consists of one soil boring, to a depth of approximately fifteen feet at a 60° angle. Mr. Feldman has completed the Forty Hour Health and Safety Training requirements. 4.5 JOB HAZARDS Chemical hazards could be encountered from direct skin contact with contaminated soils. This site will be designated as Level D. Weather conditions have the potential to disrupt the project. If it is raining the drilling and sampling will be postponed. 4 4.6 RISK ASSESSMENT The potential for adverse health or safety risks to the on-site workers or the immediate surrounding area is Iow. No unauthorized personnel will be permitted to stand near the boring locations. 4.7 PERSONAL PROTECTIVE EQUIPMENT This site will be designated as Level D for establishing the level of personal protective equipment requirements. Level D equipment includes: Safety BOots Eye Protection Protective Gloves If the need arises, half mask respirators and coveralls will be available. 4.8 GENERAL SAFETY MEASURES The following general safety procedures shall be followed by all persons entering the work site: No one will be allowed on-site without the prior knowledge and consent of the site safety officer. All personnel shall be advised of the site safety requirements as discussed in this plan. All personnel shall bring to the attention of the site safety officer any unsafe conditions. There will be no smoking or drinking in the work zone. Hands shall be thoroughly cleaned prior to smoking, eating or drinking. 4.9 PLAN ACCEPTANCE All personnel involved with this project are required to read this Health and Safety Plan and sign in the space provided that they have read this plan and understand its requirements. (3) The underground storage tank may be filled with a non corrosive liquid that is not a hazardous substance. This liquid shall be tested and the test results submitted to the local agency prior to removal from the underground storage tank at the end of the temporary closure report. (4) Except for required venting, all fill and access locations and piping shall be sealed using locking caps or concrete plugs. (5) Power service shall be disconnected from all pumps a ssociated with the use of the underground storage tank unless the power services some other equipment which is not being closed, such as the impressed-current cathodic protection system. (b) The monitoring required pursuant to the permit may be modified by the local agency during the temporary closure period. In making a decision to modify monitoring requirements, the local agency shall consider the need to maintain monitoring in order to detect unauthorized releases that may have occurred during the time the underground storage tank was used by that have not yet been detected. In all cases, corrosion protection shall continue to be operated. (c) The underground storage tank shall be inspected by the owner of operator at lease once every three months to verify that the temporary closure measures are still in place. The inspection shall include but is not limited to the following: (1) Visual inspection of all locked caps and concrete plugs. (2) If locking caps are used, at least on shall be removed to determine if any liquids or other substances have been added to the underground storage tank or if there has been a change in the quantity or type of liquid added pursuant to subsection (a) (3) of this section. (d) At the end of a temporary closure period over 12 months, including any extension granted by the local agency, the owner may reuse the underground storage tank only it the tank meets the requirements of Article 3 for new underground storage tanks or is upgraded to meet the requirements of Article 6. (e) All new and existing underground storage tank systems which have been temporarily closed must continue to comply with repair and recordkeeping requirements, release reporting and investigation requirements, and release response and corrective action requirements specified in this chapter and Chapter 6.7 of the Health and Safety Code. Failure to comply with the requirements will immediately terminate the temporary closure status. Upon termination of temporary closure, either through non-compliance or by expiration of the 12 month term, the tank(s) must either be integrity tested and repermitted for operation or else removed from the site under a valid permit for permanent closure. California Avenue. This tank currently comes under existing State law for Removal Or Abandonment'In Place since it fits the following criteria: 1. The tank is empty. 2. The tank has a fill pipe. 3. The capability to dispense from the tank ~till exists. To remove the tank, core samples must be taken underneath the tank ~o check for contamination and the tank must be removed from the ground and properly disposed of. This could be the most costly of our two options. The other option is to' fill the tank in place with two-sack concrete. This abandoning in place process is not utilized by many cities in California nor in Bakersfield as it could pose significant additional removal costs if development in the area of the tank takes place at a later day. Even with this option, soil tests still must be taken directly under the tank to check for contam£nation. We are currently exploring o~tions available to a~sis~ ~he property owner in handling this situation. We have ~he option to issue the owner a one-year temporary Closure PermlK. This would provide him more time to make arrangements for removal and secUre a funding source ~n order to proceed with the work. There are some fairly iow interest State loans available to pursue this option. We are also researching the least costly soil testing process directly under the tank. With a negative soil sample result (which should be the case) perhaps we can seal the fill pLpe and extend the time frame for dealing with the specific problem. California Avenue, Star Transmission, Edward Wolfe, Property Owner On October 30, 1995, I noticed a gas pump dispenser island and vent pipe against the North fenceline of this property while driving in the area. I looked up the property ownership and sent a standard Notice Of Violation for an Abandoned Underground Storage Tank. The Notice of Violation was returned for new address and was resent on November 28, 1995. On November 30, 1995, Mr. Wolfe called me regarding the Notice Of Violation. ^ record of the conversation is attached. On December 5, 1995, Mr Wolfe visited our office in order to obtain a removal permit, but left when he was advised of the cost of the permit because he '1orgot his checkbook". He never returned. On December 12, 1995, I returned a call from Vernon Strong 'on behalf of Mr. Wolfe. A record of the conversation is attached. On January 3, 1996, I met both Mr. Wolfe and Mr. StrOng at the tank site. I told them that if the tank had been "decommissioned", then no further action would be required. However, during our inspection, we could not find the tank inlet to determine if the tank had been previously "decommissioned". Mr. Wolfe told me that during his 30 years of owning the site, he had never used the tank. During this tank inspection, I noted that a new hazardous material handler was occupying the site. I returned to the office and discussed the situation with you, upon which we both agreed that it had to be conclusively determined whether the tank was "decommissioned". I returned to the site approximately two hours later. I assisted the new business owner in completing the hazardous materials business plan and while I was there, continued to look for the tank inlet. I found the inlet under about 1" of dirt and debris. I opened the tank, probed it, and found it to be dry but not filled or sealed in any other way. I returned to the office and informed you of my finding the tank inlet and the tank's condition. On January 4, 1996, I telephoned both Mr. Wolfe and Mr. Strong and notified them of my subsequently finding the tank inlet later the previous afternoon. I advised that the condition of the tank did not qualify it as being "decommissioned" and that the removal requirement may still apply. Mr. Strong questioned the determination and suggested that he would take the matter up with the appropriate council member and yourself. Attached, here also, is the guidance letter LG-122 concerning closure regulations to tanks decommissioned before January 1, 1984. The tank has the capability of being used in it's present condition because the inlet is accessible, the tank is empty and not filled with an insert solid and the dispenser island has piping and electrical connections present and accessible.
