Dear Mr. Ruettgers:
This department has received all the required submittals for
the tank closures at the above addresses, Permit numbers A736-16
and A765-16. The Laboratory analyses for the soil samples taken'
beneath the three tanks remowad at these sites indicated no
significant soil contamination.
Based upon this information, the department considers.these
preliminary assessments to complete and no further tank assessment
is required.

Dear Sir:
Forwarded herewith are the .laboratory test reports for the preliminary
assessment soils testing at the above referenced facility. As you will
note all parameters were reported below the detection level.
Previously forwarded, in conjunction with the tank demolition at
Valley Tree and Construction facilities, were:
a. Copy of hazardous waste manifest #87152816 to cover the.
removal and disposal of tank contenl:s at Gibson Refinery in accordance with
recycling instructions.
b. Copy of non-hazardous waste manifest from MP Vacuum Service
Company for waste certified non-hazardous and required test reports.
c. Copy of non~hazardous manifest for tanks demolished at the
Valley Tree & Construction facilities.

1. Record the date meter was last calibrated by a certified
technician from the 'calibration' tag .attached to the unit.
2. Check the cotton 'wad' in the wand and replace with a
clean 'wad' if there is evidence of dirt or odor; Place the
wand on the instrument. Cotton replaced? Yes or no
3. Turn the power switCh'on' by depressing the 'Power On'
button and reading the 'percent of charge' from the meter. The
minimum 'percent charge' for use is seventy percent (70%).
Record the 'percent charge' from the meter reading
4. With the 'Oxy in / LEL out' button in the 'out' position
wait 30 seconds after energizing the unit and calibrate the
LEL.. Calibrate by adjusting the 'LEL zero' knob so that the
meter reads zero. While calibrating be sure that the wand is
only exposed.to clean air. LEL zero? Yes or no
5. With the 'Oxy in / LEL out' button in the 'in' position
wait 30 seconds after energizing the unit and calibrate the
percent oxygen. Calibrate by adjusting ·the 'Oxy Cal' knob
so that the needle on the meter reads 21%. This is shown on
the face of the meter with a long line marked 'cal'. While
calibrating insure that wand is only exposed to clean air.
Is 'percent oxygen' calibrated to the mark? Yes or no.
6. Inspect entire meter for evidence of damage, wear, or any
malfunction. Recalibrate the'lower explosive limit and oxygen
percentage again. If the recalibration is substantially dif-
frent from the original calibration determine the cause and
repeat the entire procedure until the calibrations do compare.

1. It is .the responsibility of the Permittee to obtain permits which may b
required by other regulatory agncies prior to beginning work.
2. .Permittee must obtain a City Fire Department permit prior to initiatin
closure action.
3. A minimum of four samples must be retrieved one-third of the way in fro~
the ends of each tank at depths of approximately two feet and six feet.
4. All leaded/unleaded gasoline samples must abe analyzed for benzene
toluene, x¥1ene, and total petroleum hydrocarbons.
5. Copies of transportation manifests must be submitted to the Healt
Department within five days of waste disposal.

notified before moving and/or disposing of any contaminated soil.
7. Permtttee is responsible for making sure that "tank disposition tracki;
record" issued with this permit is properly filled out and returned with_
14 days of tank removal.
8. Advise this office of the time and date of the proposed sampling With
hours advance notice.
9. Results must be submitted to this office within three days of anaiys.
