The following code violations were observed: A-Deck (Second Floor) Video Room 1. Smoke detector must be minimum of 3' from air vents 2. Provide cover to ceiling electrical J/Box. 3. Exit signs in conidor leading from jail lobby not working. B-Deck (Third Floor) No deficiencies noted. C-Deck (Fourth Floor) Mental Health Office 1. Fire extinguisher needs servicing. Basement Kitchen area 1. Missing 1 sprinkler head in existing kitchen storage room northwest corner over air duck system. 2. Housekeeping needed in dry storage room. Isles must be kept clear. 3. Center Stairwell 1. Replace corroded sprinkler head at bottom of landing 2. Provide overhead J/Box cover at same location. Maintenance Area 1. Re-attach sprinkler pipe to storage room and provide sprinkler head coverage. 2. Provide sprinkler coverage in maintenance office. 3. Housekeeping is needed in maintenance workshop are. (Sawdust, old pieces of pipe etc) 4. Remove old paint cans. 5. Provide a paint cabinet and store all paints at same location. 6. Provide a four foot clearance in front of main electrical switch board panel. Identification Room 1. Provide cover to electrical J/Box on wall. Roof 1. Replace fire extinguisher in elevator room (needs servicing). ' 2. Replace flexible seal tight conduit over video room (broken). These violations must be corrected by September 9, 2002. If contractor availability or logistic problems prevent completion by this date you must schedule an office hearing to work out an acceptable time frame for completion. In addition to the above corrections, the following items are required: 1. Building sprinkler system must have a five year inspection done, with all deficiencies corrected by a certified C-16 licensed contractor. (provide documentation) 2. Provide a complete report of an annual fire alarm test by a company or person who is trained or certified to provide such service on your present alarm system. 3. Provide individual floor plans of all fire alarm devices and their locations. 4. Provide V.L. listed document of monitoring company. (A Certificate of Compliance must be filled out by installing company) 5. R.O.E. fire protection last annual service report dated December 10. 2001 does not show a sprinkler water flow test. The management for all high-rise buildings shall establish and maintain a written fire and life safety emergency plan which has been approved by the City Fire Chief (California Building Code 3418.1.25). Provide certified documentation that the emergency power system has been inspected and tested by qualified personnel. No records or documentation for maintenance and testing of stairwells, exit corridors, or elevator pressurization systems. Action: a. Provide documentation by a certified engineering company that the smoke control system is operational and meets the requirements in the 1994 N.F.P.A 92A, Smoke Control System, Test and Maintenance Procedures (if applicable). RECOMMENDATIONS FOR KEEPING RECORDS 1. Keep all test documents in one three-ring bindér, with dividers for each system to be tested and maintained. This will expedite and maintain good control of all documents when various inspection times roll around. 2. Keep maintenance personnel trained in operational procedures for testing of: a. Emergency power generator system. b. Fire pump system. c. Smoke control pressurization system. 3. Attached is a list of deficiencies found at various locations of your facility.
