RE: Fire Prevention Inspection of 1415 Truxtun Avenue
Notice of Violation
As the fire prevention agency having jurisdiction, we conducted an
inspection of the Kern County Centra] Receiving Facility in July of 2002.
The following code violations were observed:
A-Deck (Second Floor)
Video Room
1. Smoke detector must be minimum of 3' from air vents
2. Provide cover to ceiling electrical J/Box.
3. Exit signs in conidor leading from jail lobby not working.
B-Deck (Third Floor)
No deficiencies noted.
C-Deck (Fourth Floor)
Mental Health Office
1. Fire extinguisher needs servicing.
Basement
Kitchen area
1. Missing 1 sprinkler head in existing kitchen storage room
northwest corner over air duck system.
2. Housekeeping needed in dry storage room. Isles must be kept
clear.
3. Northeast kitchen hallway exit door to parking area is missing
sprinkler head coverage.

Receiving Control Room
1. Replace painted sprinkler head.
Center Stairwell
1. Replace corroded sprinkler head at bottom of landing
2. Provide overhead J/Box cover at same location.
Maintenance Area
1. Re-attach sprinkler pipe to storage room and provide sprinkler
head coverage.
2. Provide sprinkler coverage in maintenance office.
3. Housekeeping is needed in maintenance workshop are.
(Sawdust, old pieces of pipe etc)
4. Remove old paint cans.
5. Provide a paint cabinet and store all paints at same location.
6. Provide a four foot clearance in front of main electrical switch
board panel.
Identification Room
1. Provide cover to electrical J/Box on wall.
Roof
1. Replace fire extinguisher in elevator room (needs servicing). '
2. Replace flexible seal tight conduit over video room (broken).
These violations must be corrected by September 9, 2002. If contractor availability or
logistic problems prevent completion by this date you must schedule an office hearing to
work out an acceptable time frame for completion.
In addition to the above corrections, the following items are required:
1. Building sprinkler system must have a five year inspection done, with
all deficiencies corrected by a certified C-16 licensed contractor.
(provide documentation)
2. Provide a complete report of an annual fire alarm test by a company or
person who is trained or certified to provide such service on your
present alarm system.
3. Provide individual floor plans of all fire alarm devices and their
locations.
4. Provide V.L. listed document of monitoring company. (A Certificate
of Compliance must be filled out by installing company)
5. R.O.E. fire protection last annual service report dated December 10.
2001 does not show a sprinkler water flow test. Explain how water
flow is being monitored by fire alarm panel.

Thank you for choosing SimplexGrinnell as the provider of your Fire Alarm Inspection senices,
which were completed on September 27, 2002.
REPORT COMMENTS
Fire Alarm Speakers were tested and all worked properly.
Door Holder Smoke Detectors tested on 1st, 3rd and 4th floors did not alarm Fire Alarm Panel, did not
release doors - several would not hold doors open - some doors are held open by trash cans or wedges.
Basement Mechanical Room North, Basement Storage Room, 1st Floor Mechanical Room North and 4th
Floor Mechanical Room East Smoke Detectors failed to alarm when tested.
1st Floor Pull Station by South Stairs had a piece of plastic, instead of break glass - plastic would not
break to allow access to Pull Station - removed plastic piece.
Pull Station Housings missing Break Glass: 1st Floor by South Stairs, 2nd Floor by East Stairs, 4th Floor
by Public Elevator.
Pull Station Housings missing screw: 2nd Floor by Public Elevator, 2nd Floor by West Stairs.
2nd Floor by Public Elevator Phone Jack is missing Ring Nut - will not stay in place.
3rd Floor by Freight Elevator Phone Jack is blocked by sbelving unit.

This confirms the contents of our phone conversation iTom this morning.
¡,n July of this year we perfonned a 5 year inspection and testing of the fire sprinkler
system at the downtown jail facility at 1415 Truxton Ave. Bakersfield. At that time I
spoke with you about some relatively minor corrections that needed to be done per fire
codes and assured you that our company would be correcting the deficiencies very
shortly. It has now been almost three months and we have made repeated attempts to
make the corrections but have been prevented or delayed each time by a Sheriffs
Department employee, Rhonda Turnbaugh. We have had for some time a purchase order
from both the County ofKem General Services and Sheriffs Department authorizing the
repairs our most recent attempt to make the corrections was a scheduled appointment
with Ms. Turnbaugh for yesterday, October 1.2002. We sent two of our men prepared to
begin work immediately but were again stalled by Ms. Turnbaugh. That was when 1
called your office to inform you of this matter, both in the interest of public safety and
legally protecting our firm. You were kind enough to retUrn my ca)J this morning at
which time we discussed this matter.

The following items were noted during the annual inspection by the Fire
Prevention Office, Bakersfield City Fire Department, on September 22, 1999.
This report addresses record keeping and deficiency items found during the
inspection of the seven-floor high-rise building.
All deficiency items mentioned in this report are covered by the 1995
California Fire Code, 1995 California Building Code, 1994 N.F.P.A
Standards for Testing and Maintenance Procedures, and Title 19 California
Code of Regulations.
This report will, first, address record keeping; secondly, deficiencies found
throughout the building.

FIRE PUMP & RELATED EOUIPMENT
1. No records were found for the maintenance and testing of the fire pump system.
2. NOTE: Fire pump is not being tested by emergency power generator.

initiated following the 1994 N.F.P.A. 25 Standards.
Provide testing documentation certifying that the entire system is in operating
condition and meets safety requirements of all related codes.
FIRE ALARM TEST RECORDS
1. No records were found for the maintenance and testing of the fire alarm system.
2. No maintenance or quarterly records are being kept by the Maintenance Department.
Action:

Procedures for testing, maintenance, and record keeping must be immediately
initiated following the 1994 N.F.P.A. 72 Standards.
EMERGENCY EV ACUA TION PLAN
1. Emergency evacuation manual could not be located.

The management for all high-rise buildings shall establish and maintain a written
fire and life safety emergency plan which has been approved by the City Fire Chief
(California Building Code 3418.1.25).

Provide certified documentation that the emergency power system has been
inspected and tested by qualified personnel.
All items mentioned below must be in compliance with the 1994 N.F.P.A. 110,
Testing and Maintenance Procedures.
1. Emergency lighting in all exit corridors, all levels.
2. Emergency exit signs, all levels.
3. Emergency lighting in stairwells.
4. Record of emergency generator being tested under load for items
mentioned above, 1 through 3.

A
SMOKE CONTROL PRESSURIZATION SYSTEM
1. No records or documentation for maintenance and testing of stairwells, exit corridors, or
elevator pressurization systems.
Action:

Provide documentation by a certified engineering company that the smoke control
system is operational and meets the requirements in the 1994 N.F.P.A 92A,
Smoke Control System, Test and Maintenance Procedures (if applicable).
RECOMMENDATIONS FOR KEEPING RECORDS
1. Keep all test documents in one three-ring bindér, with dividers for each system to be
tested and maintained. This will expedite and maintain good control of all documents
when various inspection times roll around.
2. Keep maintenance personnel trained in operational procedures for testing of:
a. Emergency power generator system.
b. Fire pump system.
c. Smoke control pressurization system.
3. Attached is a list of deficiencies found at various locations of your facility. These items
are to be corrected and reinspected.

DEFICIENCY NOTES
1. Electrical room near Department #3: electrical panel is missing cover.
Action: Replace cover.
2. Court Room # 11: exit signs are not self-illuminated, and the emergency lights are not
adequate to supply light at the sign.
Action: Correct per code.
3. Hose cabinets: fire hose equipment is to be hydro tested every three years.
Action: Have all fire hoses tested, and provide test documentation for inspector.
4. Court Rooms #1, 2, 3,4, 5, 6, 8, 9, 111 have similar exit illumination problems as item #2
above (Court Room #11).
Action: Correct per code.
5. The hallway behind Court Rooms #7,8,9 does not have enough exit signs.
Action: Supply exit signs per code.
6. NOTE: Rated corridor doors are to be closed when exiting into a rated corridor.
Action: Remove all door stops.
7. First and second floors do not have any emergency lighting that inspector is aware of.
Action: It: when load testing emergency generator, there is no light in these areas, means
of illumination will need to be provided.
8. Seventh floor District Attorney's Office: corridor interior rated door is missing closing
hardware.
Action: Repair, or provide as needed.
9. Mount fire extinguisher in elevator room (maximum height five feet).
10. Mount fire extinguisher in service room near elevator access (maximum height five feet).
11. No exit signs in Probation offices (#603).
Action: Provide as per code.
12. Fourth floor east wing stairwell hose outlet missing valve handle.
Action: Provide proper handle.
13. Second floor , Clerk's office contains four rated roll-down fire doors; they should be tested.
(Speed of descent for roll-down doors should not exceed two feet per second.)
Action: Provide test documentation.
