CONDITIONS
'1. The vapor recovery systems and their components shall be installed, operated, and maintained in accordance with the
State certification requirements. [District Rules 4621 and 4622]
2, This gasoline storage and dispensing equipment shall not be used in retail sales, where gasoline dispensed by the unit
is subject to payment of California sales tax on gasoline sales. [District Rule 4622]
I 3. No gasoline delivery vessel shall be operated or be allowed to operate unless valid State of Cali fomi a decals are
displayed on the cargo tank which attest to the vapor integrity of the tank. [District Rule 4621]
'4. Vapor recovery systems and gasoline dispensing equipment shall be maintained leak-free. A "leak" is defined as the
dripping ofliquid volatile organic compounds at a rate of three or more drops per minute, or vapor volatile organic
compounds in excess of 10,000 ppm as equivalent methane as detennined by EP A Test Method 21. [District Rule
4622]
5. Any person conducting tests shall have completed a District-approved training program or the District's orientation
class for testing and any subsequent required refi'esher class. [District Rule 4622]

Approval or denial of a PERMIT TO OPERATE will be made after an inspection to verify that the equipment has been constructed in accort!ance with the
approved plans, specifications and CCJndjtions of this Authority to Construct, and to determine if the equipment can be operated in compliance with an

,6. For certified Phase II vapor recovery systems with liquid r:emoval devices, the pennittee shall perfonn and pass an
ARB TP-20 1.6 Liquid Removal Test within 60 days after initial start-up and whenever the liquid in the vapor path
exceeds 100 ml of liquid. The amoWlt ofliquid in the vapor path shall be measured by lowering the gasoline
dispensing nozzle into a container until such time that no more liquid drains from the nozzle. The amount of liquid
drained into the container shall be measured using a graduated cylinder or graduated beaker. The vapor path shall be
inspected once per month if monthly throughput is below 2,500 gallons, or once per week otherwise. [District Rule
4622]
7. The permittee shall perfOl1'Il and pass a Static Leak Test for AbovegroWld Tanks using ARB TP-201.3B within 60 days
after initial start-up and at least once every 12 months thereafter. [District Rule 4622]
, 8. The operator shall implement a periodic' maintenance inspection program for the certified Phase II vapor recovery
system consistent with Section 5.4.2 of Rule 4622. The program shall be documented in an operation and maintenance
(O&M) manual and shall at aminimum contain the following infonnation: 1) All applicable ARB Executive Orders,
Approval Letters, and District Permits; 2) The manufacturer's specifications and instructions for installation, operation,
repair, and maintenance required pursuant to ARB Certification Procedure CP-20 1, and any additional instruction
provided by the manufacturer; 3) System and/or component testing requirements, including test schedules and passing
criteria for each oft..~e standard tests., The owner/operator may include any non-A.'1U3 required di2.gIlostic and other
tests as part of the testing requirements; 4) Protocol for performing periodic maintenance inspections including the
components to be inspected and the defects requiring repair; and 5) Additional O&M instructions, if any, that are
designed to ensure compliance with the applicable rules, regulations, ARB Executive Orders, and District permit
conditions, including replacement schedules for failure or wear prone components. [Di~trict Rule 4622]
9. The operator shall conduct periodic mairitenance inspections based on the amoWlt of gasoline dispensed by the facility
in a calendar month as follows: A) less than 2,500 gallons - one day per month; B) 2,500 to less than 25,000 gallons -
one day per week; or C) 25,000 gallons or greater - five days per week All inspections shall be documented within the
o & M Manual. [District Rule 4622]
I 10. The operator shall maintain monthly gasoline throughput records. [District Rule 4622]
11. All records required by this permit shall be retained on-site for a period of at least five years, and shall be made
available for inspection upon request. [District Rule 4622]
: 12. The operator shall maintain on the premises a log of any repairs made to the certified Phase lor Phase II vapor
recovery system. The repair log shall include the foHowing: 1) date and time of each repair; 2) the name of the
person(s) who perfonned the repair, and if applicable, the name, àddress and phone number of the person's employer;
3) description of service performed; 4) each component that was repaired, serviced, or removed; 5) each component
that was installed as replacement, if applicable; and 6) receipts or oth~ documents for parts used in the repair and, if
applicable, work orders which shall'include the name and signature of the person responsible for performing the
repairs. (District Rule 4622)
, 13. The District shan be notified by the permittee 15 days prior to each test. The test results shall he submitted to the
District no later than 30 days after each test. [District Rule 1081]
14. The District shan be notified within 24 hours of the facility's pass/fail status after the perfonnance of each test.

INTRODUCTION: The purpose of this HASP is to set forth,in and orderly and logical fashion,
appropiate health and safety procedures to be followed during onsite construction activities at
the Three -Way Chevrolet dealership.
During the performance of the task to be performed, this HASP identifies potential hazards which
DI Maggio Maintenance personnel may be exposed to DI MAGGIO MAINTENANCE personnel shall not
participate in this TASK without having read this plan in it's entirety. This plan has been developed to be
as complete as possible, however, should conditions dictate revisions or additions to this plan,
amendments shall be drafted, added, and distributed to all persons involved with this plan.This plan
works in concert with the DI MAGGIO MAINTENANCE Corporate Safety and Health Plan,Enviromental
Protection Agency regulations,AII Fire Protection Association Codes, OSHA Standards, and any other
applicable codes.
STATEMENT OF COMPANY'S SAFETY GOALS AND OBJECTIVES:
It is the goal of DI MAGGIO MAINTENANCE and all of it's employees to obtain and maintain a
safe jobsite from the start of the project to the completion without incident or injury.
Objectives: 1. Work in the most safe and efficient manner possible.
2. Make all personnel on site aware of any possible safety issues.
3. Hold and record daily tailgate safety meeting prior to job start.
4. Make sure all employees are supllied and utilizing all necessary safety equipment.
5. Employee are to be knowledgable in the task they are to perform.
6. Make sure every employee is able to go home every day without incident or injury.
DESCRIPTION OF WORK.:
Di Maggio Maintenance has been contracted to install a EV2500- 2500 Gallon Above Ground Storage
Tank for the non-retail dispensing of Gasoline at Three -Way Cadillac- 4600 Wible Road Bakersfield,CA
A seismically safe concrete pad will be installed by concrete contractor for tank. All crash posts will
be installed per Fire Code Regulations.AII Electrical will be performed by a licensed electrical contractor
who will be pulling the electrical permit. Permitting for Bakersfield Fire, Air Board, and Water Board
will be obtained by Di Maggio Maintenance. Installation perfromed to all pertinent codes and manufacture
specifications.
Employee Awareness of Safety:
Di Maggio Maintenance number one goal is perform all job activities in the safetest manner possible..
This goal is achieved by holding monthly safety meetings at our office with all employees.
Every crew holds a tailgate safety meeting prior to any job start.
We offer dinner certificates for our employees who have a month of no incidents, accidents or violations.
Competent Persons:
