Accordingly, it is important that each individual recognize and obey certain rules when doing their job. The rules included in this manual are basic and general in nature and cannot cover every possible working condition. Additional local rules and procedures may apply in certain circumstances. You should consult with a T.D. Whitton Construction, Inc. Supervisor, on safety procedures before proceeding with your work. The cooperation of each employee is a necessity so that operating procedures and work methods do not expose you or your co-workers to injury. Remember that safety is a team effort in which each individual must share to eliminate or reduce the risk of loss or damage to personnel or property. Good judgment and common sense are required to supplement any rules. T.D. Whitton Construction, Inc. intends to maintain an independent relationship with each employee while insisting on good safety performance. If you are in doubt at any time about safety, consult your employer. T.D. Whitton Construction, Inc. believes that all activities should be governed by the principles on the following pages and expects each person working for T.D. Whitton Construction, Inc. to understand and accept them. 1 ; WORKPLACE SAFETY PRINCIPLES 1. All work can be done safely. 2. All accidents and injuries can be prevented. 3. WHITTON CONSTRUCTION, RESPONSIBILITIES INC. SUPERVISORY 1. Marty Whitton of T. D. Whitton Construction, Inc. is responsible for seeing that proper instructions, equipment, training and guidance in correct work procedures are provided to each employee. All employees will be trained initially, and thereafter, all new employees, and all employees given a new job assignment, and when new substances, processes, procedures, or equipment are introduced to the workplace. Labor Code 6401.7(1) and 6401.7(4) 2. Unless otherwise specified, T.D. Whitton Construction, Inc. is responsible to provide you with tools and equipment including personal protective equipment which are appropriate for the purposes intended. Employees will replace any tools and equipment which is lost by them. New tools and equipment will be provided to any employee upon the return of the broken tools and equipment. Labor Code 6401.7(3) 3. T. D. Whitton Construction, Inc. supervisor, Marty Whi tton, and onsi te foreman are responsible for enforcing all rules which are contained in this manual and all regulations and rules of your employer which apply to the operation for which your supervisor is responsible. Labor Code 6401.7(6) 4. Martha Tucker of T. D. Whitton Construction, Inc. has the responsibility to provide T.D. Whi tton Construction, Inc. and our clients with a written report of any accident or injury to contract personnel as soon as possible following the accident (within 24 hours). Labor Code 6401.7(2) 5. T.D. Whitton Construction, Inc. field supervisors are responsible for safely planning the work. Should an unexpected hazard develop during the course of the job, the supervisor has the responsibility to stop work until the hazardous condition can be corrected. Should any unsafe condition be discovered, corrections will be made immediately, when possible, by company personnel or by outside contractors when 3 · company personnel are not qualified. Code 6401.7(2) and 6401.7(3) Labor 6. T.D. Whitton Construction, Inc. field supervisors are responsible for holding "tailgate safety sessions" at least every 10 working days. Labor Code 6401.7(5) 7. EMPLOYEE RESPONSIBILITIES 1. Each employee must read, be thoroughly familiar with and understand the contents of this manual. Labor Code 6401.7(6) 2. Each employee shall follow safe procedures for the work being performed, conduct operations safely and immediately report any unsafe conditions or practices without fear of reprisal to his foreman, supervisor or proper authority. ,3. Oral warnings given three (3) times for failure to follow proper safety procedures - documentation to file 2. written warnings (2) - documentation to file 3. written termination notice - documentation to file Blatant disregard for company safety policy resulting in an injury can be cause for immediate dismissal. In the event an employee is terminated, he must receive his paycheck at that time. E. THEFT Incidents of confirmed theft or unauthorized removal of property belonging to T.D. Whitton Construction, Inc. will be reported to legal authorities and will result in immediate termination. F. PERSONAL PROTECTIVE EQUIPMENT (Labor Code 6401.7(6) Specific items of personal protective equipment are required apparel while on the job. In other instances, the work activity, local environment or unit policy will dictate the need for additional items of personal protective equipment. This protective equipment will be inspected daily by the user. These requirements may include, but are not limited to, the following: 1. Head Protection Hard hats are required at all times while on the job except while in vehicles or offices. 5 2. Foot Protection Appropriate safety footwear shall be worn where foot injuries by crushing, falling objects, etc., might occur. Leisure footwear, such as sandals, shall not be worn by shop personnel or by heavy equipment operators. other footwear must be approved by onsi te foreman. 3. Face and Eye Protection Protecti ve spectacles and goggles or face- shields are to be worn when performing work which could injure your eyes. Such work would include, but not be limited to, welding, cutting or when there is airborne debris due to high winds. Sunglasses are not considered goggles. 4. Hand Protection Gloves are to be worn to protect the hands against extremely hot or cold equipment, and sharp edges. 5. Respiratory Protective Equipment a. T. D. Whi tton Construction, Inc. is responsible for providing you wi th appropriate protective breathing equipment for the work to be performed. You must wear respiratory equipment whenever you enter or work in an atmosphere known or suspected to contain dangerous concentrations of gas, vapor, dust or mist or when there is a deficiency of oxygen. Work will not normally be permitted in atmospheres that are immediately dangerous to life or heal th (IDLH). Where IDLH conditions cannot be eliminated, special procedures shall be developed for the work. The procedures are required to be approved by aT. D. Whitton Construction, Inc. supervisor prior to entry into the IDLH atmosphere. b. When you are required to wear or use any breathing equipment you must have a good face seal for the equipment to be 6 .' effective. Beards and other facial hair such as long sideburns are not allowed due to the possibility a worker may be required to use a respirator at any time. c. Whenever a self-contained breathing apparatus (SCBA) is required, a standby person who is appropriately equipped and trained shall be at the worksite to give assistance in the event of emergency. For further information, please refer to General Industry Safety Order (GISO) 5144. G. CLOTHING (Labor Code 6401.7(6) Each employee shall wear clothing appropriate for the work to be performed. contaminated or oil saturated clothing shall be changed as soon as possible. Loose or frayed clothing will not be worn at any time around the shop or equipment. Sleeveless shirts, tank tops, and shorts are not permitted. Full length pants and shirts are mandatory in shop and equipment areas. Shop employees must wear long sleeved shirts. Neckties, neck chains, loose jewelry and rings shall be removed when working around moving/rotating machinery. H. PERSONAL SAFE WORK PROCEDURES 1. Housekeeping Employees are expected to keep the worksi te clean and orderly. Emergency access and egress ways shall not be obstructed. Trash and debris shall be cleared away as it accumulates (at least daily). contaminated waste shall be handled in a proper manner and taken to a proper disposal facility. 2. Slips, Trips, and Falls a. Be aware of the hazards around you taking care to walk around them or step over them if they cannot be removed. b. Report any hazards which you may find. 7 c. Do not overreach causing you to lose your center of balance. d. When the surface is wet take short steps and make wide turns at corners. Wear slip-resistant shoes if necessary. e. If you create a spill or find one, either clean it up or mark it until it can be cleaned up. f. Don't walk pockets. balance. with Keep your your hands hands in your free for g. Lifting and Carrying To protect your back always use proper techniques to lift and carry. a. Bend with your knees and hips keeping your back straight. Use the legs to lift and lower. Do not lean from the waist. b. Do not try to lift more weight than you can easily handle and keep load close to the body. c. Wear protective gloves if necessary. d. Keep your back straight when carrying a load. Maintain good balance by counterbalancing your load by shifting part of your body in the opposite direction. 4. Repetitive Motion Injuries a. To avoid wrist and hand injuries caused by repeti ti ve motion choose tools that fit your hand, are properly shaped, and produce ~he least amount of vibration. b. stand above the object. 8 c. Use as much of your hand as possible reducing finger strain, keep your wrist straight, and try alternating hands when one becomes tired. d. Reduce elbow and shoulder injuries by keeping your elbows bent. This keeps loads close to your body and puts less weight and pressure on your shoulder. e. Use as much of your arm as you can while keeping your wrist straight. f. Don't lock your elbow when working over your head. By making broad arm strokes less of your forearm is used taking force off your elbow. 5. Lock-OutjBlock-Out Lock-out and block-out procedures are essential to prevent accidental movement of machinery or contact with energized electrical equipment. a. Lock-out Turn off and padlock the MAIN energy source. Attach a tag with pertinent information concerning the Lock-out to the padlock. b. Block-Out Block equipment that could fall, slide, roll, descent, rotate, or otherwise move. Use blocks, chains, or other equipment of sufficient strength to support or stop the movement. 6. Electrical Safety a. Be certain when serv1c1ng, maintaining, or adjusting electrical equipment that the switches are opened, locked-out and tagged. b. Portable tools and equipment must be grounded or of the double insulated type. c. Extension cords must have a grounding conductor. 9 " d. Never use multiple plug adaptors or flexible cords and cables that have been altered. e. Check all cord, cables, etc., for any exposed wiring or deterioration of any type. f. Do not use metal ladders, metal measuring tapes, ropes, etc., with metallic thread where they can come in contact with energized parts of equipment or circuit conductors. g. Do not block labels on disconnecting switches and circuit breakers. h. Always open disconnecting means before fuses are replaced. i. Take care not to block mqtor disconnecting switches or circuit breakers. j. In wet or damp locations, make sure that electrical tools and equipment are appropriate for the use or locations or otherwise protected. k. Watch for standing water when working with electrical equipment. 7. Compressed Gas Cylinders a. Do not block identifying label on cylinders. b. store cylinders in areas where they are protected from external heat sources, falling objects or tampering. c. When storing or transporting be sure the safety relief device is always in direct contact with the vapor space in the cylinder. Prevent cylinders from tipping, falling or rolling. d. Valve protectors should be placed on cylinders when not in use or connected for use. 10 " e. Close all valves before' moving a cylinder, when the cylinder is empty and at the completion of each job. f. Check cylinders periodically for corrosion, distortion, cracks or other defects. Be sure to inspect the bottom of the cylinder. 8. Welding and cutting a. Only those personnel who are qualified by training and experience shall be allowed to operate any welding equipment. b. Bystanders should be cautioned not to look directly at the arc. c. Metal ladders are not to be used during any welding procedures. 9. Tractor Safety Only those personnel who are qualified by training and experience shall operate tractors. Training will be annually. a. Have Roll-Over Protection (ROPS) and use seat belt. b. Reduce speed on slopes and turns; avoid driving near embankments and steep slopes. c. Keep Power Take-Off (PTO) shield in place. d. Only one person to a seat - no riders. e. Shut off engine when you leave the driver's seat. f. Hitch only to the drawbar. g. Operate equipment when well-rested and sober. 10. Graders a. Only those personnel who are qualified by training and experience shall be allowed to operate graders. 11 Co' b. Equipment operators shall use adequate respiratory protection when needed. c. Keep equipment under control at all times and keep in gear when descending grades. d. Do not drive at a speed greater than is reasonable and proper with due regard for weather, traffic, roadway characteristics, etc. e. Make sure required warnings are displayed on machine when roading. f. When scarifying across a slope, keep blade crossway and carry it low to provide some protection against tipping. 11. Skip Loaders a. Only those personnel who are qualified by training and experience shall operate skip loaders. b. All persons involved with skip loaders should be aware of underground lines. 12. Loaders a. Only those personnel who are qualified by training and experience shall operate loaders. b. Observe gauges, instruments, and warning lights to assure that they are functioning and their readings are within the operating range. c. stay in the operator's seat at all times when operating and keep your seat belt/operator restraint fastened when operating a machine. Be in control of your machine at all times. d. Operate at speeds slow enough so you have complete control at all times. e. Avoid sideslope travel whenever possible. Drive up and down the slope. In steep slope operation, do no tallow engine to over speed. Select proper gear speed before starting downslope. 12 13. Hydraulic Excavator a. Only those personnel who are qualified by training and experience shall operate heavy equipment. b. All persons involved with excavators should be aware of electrical lines. c. Never attempt to operate the excavator except from the approved operator's position. d. Do not load a truck unless the driver is in a safe place. e. Avoid swinging or extending the bucket farther than necessary in a downhill direction. f. Never exceed manufacturer's recommended bucket size. 14. Truck Safety a. Check engine oil, radiator water, and tires, lights, brakes., etc. daily. b. No truck will be driven faster than the posted speed limit - loaded or empty. c. Drivers are required to wear their seat belts. d. Passengers are not permitted without prior authorization. e. The parking brake shoüld be set and any other steps taken to keep the truck from moving before the driver leaves it unattended. f. After making an emergency stop, driver has ten minutes to put emergency warning devices. the out g. Fuses may not be attached to any part of a truck. h. Use headlights one-half hour after sunset and one-half hour before sunrise or anytime there is not enough light to see 13 clearly 500 feet away. Lights and reflectors are to be kept clean. 15. TRANSPORTATION AND SAFE FUELING By Motor Vehicle Practice defensive driving and good vehicle mainte- nance at all times. 1. Use of seat belts is required for all T.D.
