Regarding: Abandonment in Place of Existing Underground Diesel Tank
Enclosed is a copy of the drawing approved by OSHPD showing the final location of the
existing underground tank in the position that it would be shifted to. Also included is a
drawing showing the proposed location of an above ground diesel tank located at the
comer of "H" Street and 27th Street. I am requesting that the underground diesel tank
shown be allowed to be "abandoned in place" do to its close proximity (less than for feet)
to both the 500-ton cooling tower to the south and the surgical nitrogen and nitrous oxide
enclosure to the north. The supervisor for HBE (the general contractor) states that the
shoring subcontractor says that there is not enough room between the tank and the
structures to install the appropriate shoring for tank removal. Thank you for your

NOTICE OF VIOLATION & SCHEDULE FOR COMPLIANCE
Dear Mr. Singh:
Two inspections were performed at your location on Tuesday,
December 14, 2004 and Thursday, December 16, 2004. The purpose of this
inspection was to verify new spill buckets and a function test on UDC's
sensors.
However, during my inspection it was noted that dispenser #1 has
suffered damage. The J box was bent and the dispenser anchors are loose.
It was also noted that two unions are leaking directly above the shear valve.
Leaks were also noted in dispenser #3.
In addition, you must install an exterior emergency shut-off device to
conform to current code. Therefore, prior to January 18, 2005 you must make
the necessary repairs/upgrades. Failure to comply will result in further
enforcement action which could include revocation of your permit to operate.
