Corporation, Owner of the property located within proposed Vesting Tentative Parcel Map No.
10838, hereby request that the property be included within the Consolidated Maintenance District.
This request is in accordance with the requirements of Bakersfield Municipal Code Section
13.04.021.
Attached is a check in the amount of $1,508.00, to cover the costs of inclusion within the
Consolidated Maintenance District. Furthermore, attached is a computer disk copy of the
Tentative Parcel Map for your use.
We also agree towaive ourrights to any and all hearings in accordance with the Brown Act, the
Bakersfield Municipal Code andlorany other law, concerning the formation of or inclusion in the
maintenance district. I, Lester M. Huhn, President of the J.W. and Ida M. Jameson Foundation,
a Non-Profit Corporation, have signed and am returning the attached Proposition 218 ballot
(Official Assessment Ballot) indicating our consent to the assessment.

Copy of Letter Requesting Inclusion in Maintenance District and Official Assessment Ballot,

Jameson Foundation.
On November 15, 2001 a similar letter and assessment ballot signed by DeVllalt Corporation an
behalf of the Owner, and the $1,508 processing fee were submitted to the Public Works
Department in order to meet the November 15t" deadline for said submittal. ,The attached
paperwork, signed by the Owner is being submitted to replace those documents signed by DeWalt
Corporation.
