Section ~ 3.04.021., I, L. Alberto Lopez, as agent to the Developer of property located within
.proposed Vesting Tentative Parcel Map No. 11548, hereby request either inclusion into the
existing Maintenance district or creation ~f a new maintenance district for the property being
subdivided..
We have at this time included the applicable fees to cover the costs of processing related to
district inclusion will be paid, an electronic file of the Parcel Map for your use, and the signed
assessment ballot as required.
