A letter requesting a consolidated Maintenance District has been provided in the original submittal package to the Planning Dept. on November 17, 2004. This included an on ' al letter requesting a maintenance district from the applicant (see attached copy); a Proposition 218 Ba11ot (also attached), a CD of the TTM bb 1 b (copy sent to Public Works) and a check made out to the COB Maintenance District for the amount of $3,199.b0 (see attached receipt). We will submit these missing items directly to John Stinson, COB Public Works, to avoid any delay in their process of putting a maintenance district together prior to the hearing. Section B: 4.
