This assessment ballot is for the use of the property owner of the parcel(s) identified below, which
parcel(s) is proposed to be included in the City of Bakersfield Consolidated Maintenance District
(hereinafter CMD).
This assessment ballot may be used to express either support for or opposition to inclusion within
the CMD. In order to be counted, this assessment ballot must be signed below by an owner or, if the
owner is not an individual, by an authorized representative of the owner.

RE: Inclusion of Assessor's Parcel Numbers in a Maintenance District.
Gentlemen,
I, the undersigned, as owner of the property included within the following Assessor's
Parcel Numbers
.149-200-03-00-1, 149-200-09-00-9, 149-200-10-00-1, 149-200-11-00-4,
149-200-12-00-7, 149-200-13 =00-1, 149-200-1 ~-00-2,~ ~ .
hereby request that the property be included within. the Consolidated Maintenance
Listrict. This request is pursuant to the requirements of Municipal .Code
Section13.04.021. (Note, that per the Bakersfield Assessor's office, as of 5-6-2002, there
is not any Tract/Parcel Number currently assigned for this property).
I also waive our right to any and all hearings, (whether pursuant to the Brown Act, the
Bakersfield Municipal Code, or any other law), concerning the formation of, or inclusion
in a raintenance district.

Ballot), indicating my consent to the assessments.
