Inc., a California Corporation, as owner of property located within proposed Tentative
Tract Map No. 5991, hereby request the formation of a new maintenance district or
inclusion into an existing maintenance district for the property to be subdivided.
Enclosed is a check in the amount of $1760.00 to cover the costs of processing related
to district formation or inclusion.
