This assessment ballot is far the use of the property owner of the parcel(s) identified below,
which parcels} is proposed to be included in a City of Bakersfield Traffic Signal Maintenance
District thereinafter TSMD).
This assessment ballot may be used to express either support for or opposition to inclusion
within a TSMD. In order to be counted, this assessment ballot must be signed below by an
owner or, if the owner is not an individual, by an authorized representative of the owner.

Gentlemen,
We, the undersigned, as owner of the property included within (4900 Panama Lane,
Parcel 3 of Parcel Map 9918, Project Number 05-0654) hereby request that the property
be included within a Traffic Signal Maintenance District. This request is pursuant to
Resolution No. 161-94 in accordance with Municipal Code Section 13.04.
Enclosed is a check in the amount of $1,628.15 to cover the costs of inclusion within
the consolidated Maintenance District. Further, our engineer has supplied you with a
disk copy of the (4900 Panama Lane, Parcel 3 of Parcel Map 9918, Project Number 05-
0654)for your use.
We also waive our right to any and all hearings (whether pursuant to the Brown Act, the
Bakersfield Municipal Code, or any other law) concerning the formation of or inclusion of
the maintenance district.
We have signed and are returning the enclosed Proposition 218 ballot (Official
Assessme Ballot) indicating our consent to the assessments.
