Colt Construction Services Division Kern County General Services 1115 Truxtun Avenue, Third Floor Bakersfield, CA 93309-4639 Re: Boot Failure on Secondary Containment on UST System Located At 1415 Truxtun Avenue Dear Mr. Colt: On Monday, January 24, 2005, at 11:00 a.m. I was asked to meet Ms. Elisa Redfearn of Diamond Resources and yourself at the above-referenced site. The purpose of this site visit was to verify a failed boot located just above the transition from underground piping to aboveground steel pipe adjacent to the aboveground day tank. My observations are as follows: The boot itself showed no evidence of cracking or material defect. The clamps and position of the boot are consistent with proper manufacturers placement to ensure a tight fit (Secondary Piping Inspection 11/10/03) (Final 05/13/04) The boot was found to have a'/a" tear along the side. It was also observed that a scrape extends approximately 1-1/4" on either side of the tear. It is very apparent that something external impacted the boot. It is recommended in the future that personnel having access to that area use extreme caution when moving equipment in and out of that area. Once (receive athird-party certification that the repair has been made and the boot tested, this office will certify you to be in compliance.
