Dear Ms. Bonesteel,
am in receipt of your letter of April 12, 2011, addressed to the City of
Bakersfield, whereby you explained that Southwest Health Care is no
longer in business at the above location as of March 1, 2010.
This is to inform and assure you that the above referenced account, an
annual hazardous materials and waste permit fee in the amount of $217,
has been processed for closure with no fee amount due or pending.
I apologize for the length of time it may have taken to process your prior
notifications by phone throughout these past several months, and for the
inconvenience of your having received any additional past due statements
during the interim.
In token of my sincerity, I have attached photocopies of the actual
Adjustment forms I have processed through the City Finance Department,
subsequent to your earlier phone messages, for your benefit.
As stated to you initially by our Hazardous Materials Specialist, Craig
Perkins, no payment on this particular statement of account is required.
